      标题:低温等离子鼻甲射频消融术联合鼻内镜对慢性鼻-鼻窦炎患者鼻通气及鼻腔黏液纤毛清除能力的影响

      【摘要】 目的 探讨低温等离子鼻甲射频消融术联合鼻内镜对慢性鼻-鼻窦炎(CRS)患者鼻通气及鼻腔黏液纤毛清除能力的影响。方法 将东莞市企石医院耳鼻喉科2014年3月至2016年3月收治的96例CRS患者采用随机数表法分为观察组和对照组各48例,观察组患者行低温等离子鼻甲射频消融术联合鼻内镜及下鼻甲骨折外移术,对照组患者则行鼻内镜及下鼻甲骨折外移术。比较两组治疗效果及患者在治疗前及治疗12个月后的主观症状、鼻通气、嗅觉功能、鼻腔黏液纤毛清除能力的差异。结果 观察组患者的治疗总有效率为87.5%,明显高于对照组的66.7%,差异具有统计学意义(P<0.05);术前,两组患者在鼻通气、嗅觉功能及鼻腔黏液纤毛清除能力等方面比较差异均无统计学意义(P>0.05);术后 12个月,观察组患者的嗅觉评分、糖精清除时间分别为(12.95±1.38)分、(20.08±3.23) min,明显低于对照组的(11.55±1.44)分、(25.17±4.19) min,鼻呼吸影面积、鼻腔黏液纤毛清除率及黏液纤毛清除速度分别为(10.89±1.05) cm2、(79.39±6.27)%、(8.98±0.43) mm/min,明显高于对照组的(10.42±1.07) cm2、(70.15±7.19)%、(6.19±0.33) mm/min,差异均有统计学意义(P<0.05)。结论 低温等离子鼻甲射频消融术联合鼻内镜能够明显改善CRS患者鼻通气及鼻腔黏液纤毛清除功能,治疗效果显著。

Effect of low temperature plasma turbinate radiofrequency ablation combined with endoscopic sinus surgery onnose ventilation and nasal mucous ciliary clearance capacity of chronic rhinosinusitis patients.

【Abstract】 Objective To investigate the effects of low temperature plasma turbinate radiofrequency ablationcombined with endoscopic sinus surgery on nose ventilation and nasal mucous ciliary clearance capacity of chronic rhino-sinusitis (CRS) patients. Methods A total of 96 patients with CRS treated in Department of Otolaryngology, DongguanQishi Hospital from March 2014 to March 2016 were randomly divided into observation group (n=48), and control group(n=48). The observation group was treated with low temperature plasma radiofrequency ablation combined with endo-scopic sinus surgery and inferior turbinate fractures surgery, and the control group underwent endoscopic sinus surgeryand inferior turbinate fractures surgery. The differences of subjective symptoms, nasal ventilation, olfactory function andnasal mucociliary clearance capacity were compared between the two groups before treatment and 12 months after treat-ment. Results The total effective rate of treatment in the observation group was 87.5%, which was significantly higherthan 66.7% in the control group (P<0.05). There was no significant difference in nasal ventilation, olfactory function andnasal mucociliary clearance capacity between the two groups before operation (P>0.05). Twelve months after operation,the olfactory score and saccharin clearance time in the observation group were (12.95±1.38), (20.08±3.23) min, whichwere significantly lower than (11.55±1.44), (25.17±4.19) min in the control group. The nasal respiratory shadow area, na-sal mucociliary clearance rate and mucociliary clearance rate in the observation group were (10.89±1.05) cm2, (79.39±6.27)%, (8.98±0.43) mm/min, which were significantly lower than (10.42±1.07) cm2, (70.15±7.19)%, (6.19±0.33) mm/minin the control group (P<0.05). Conclusion Low temperature plasma turbinate radiofrequency ablation combined with en-doscopic sinus surgery can significantly improve nasal ventilation and nasal mucociliary clearance in CRS patients.
