标题:RI值及弹性模量值在乳腺BI-RADS分类中的诊断效能

【摘要】 目的 比较阻力指数(RI)及弹性模量值在乳腺BI-RADS分类中的诊断效能。方法 回顾性分析广东省第二中医院2017年10月至2018年4月经手术病理证实的65例患者的临床资料,共包括70个病灶,上述病灶在术前均进行了彩色多普勒检查及剪切波弹性成像(SWE)弹性模量值测定。分别以病灶内部血流的RI≥0.70及最大弹性模量值≥80 kPa为界点,把病灶分类为BI-RADS 4类以上;以病理诊断结果为金标准,对比分析RI值及弹性模量值在乳腺BI-RADS分类中的诊断效能。结果 70个病灶中良性病灶39个,恶性病灶31个。单用RI值≥0.70分类BI-RADS 4类以上的敏感性为87.8%,特异性为84.0%;单用最大弹性模量值≥80 kPa分类BI-RADS 4类以上的敏感性为88.1%,特异性为83.5%。结果显示,RI值及弹性模量值方法的诊断敏感性和特异性比较差异均无统计学意义(P>0.05)。结论 RI值及弹性模量值在乳腺BI-RADS分类中的诊断效能相当。

【Abstract】 Objective To compare the diagnostic efficiency of RI and elastic modulus in BI-RADS classifica-tion of breast. Methods Retrospective analysis of 70 lesions confirmed by operation and pathology from 65 patients inMedical Technical Department, the Second Hospital of Traditional Chinese Medicine of Guangdong from Oct. 2017 toApr. 2018 was performed. All lesions were examined by color Doppler and shear wave elastography (SWE) elasticmodulus before operation. The lesions were classified into more than 4 groups with the resistance index (RI) of the internalflow of the lesion no less than 0.70 and the maximum modulus of elastic modulus no less than 80 kPa as the boundarypoints. The pathological diagnosis results were the gold standard, and the diagnostic efficiency of the RI value and themaximum modulus of elasticity in the BI-RADS classification of the mammary gland was compared and analyzed.Results There were 39 benign lesions and 31 malignant lesions in the 70 lesions. The sensitivity of the single use of RI≥0.70 BI-RADS 4 was 87.8%, and the specificity was 84.0%. The sensitivity was 88.1%, specificity was 83.5% by Maxi-mum modulus of elasticity, versus 88.1%, 83.5% for elastic modulus of elasticity (P>0.05). Conclusion The diagnosticvalue of RI and elastic modulus in breast BI-RADS classification is similar.
