      标题:癌症患者疾病进展恐惧现状及其影响因素

      【摘要】 目的 了解癌症患者疾病进展恐惧程度(FoP)的现状,并分析其影响因素,为完善临床癌症患者个性化的护理提供参考。方法 采用便利抽样法,选取2017年8月至2018年2月在广西医科大学第一附属医院肿瘤内科就诊的 152例癌症患者,应用自行设计的一般资料调查表、疾病进展恐惧简化量表(FoP-Q-SF)、医学应对问卷(MCMQ)进行调查。采用多元逐步回归模型分析影响癌症患者疾病进展恐惧的相关因素。结果 本组152例癌症患者疾病进展恐惧的得分为(30.07±9.68)分,处于低等水平 43例(28.3%),中等水平 64例(42.1%),高等水平 45例(29.6%);单因素分析结果显示,不同性别、年龄、居住地、医保类型和目前工作状态患者,疾病进展恐惧水平比较差异均有统计学意义(P<0.05);多元逐步回归分析结果显示,性别、年龄、居住地和医保类型是癌症患者疾病进展恐惧的影响因素(P<0.05);癌症患者应对方式的得分分别为面对(18.26±1.17)分,回避(14.80±1.76)分,屈服(11.98±1.13)分;疾病进展恐惧与回避呈显著正相关(r=0.223,P?0.01),与屈服呈负相关(r=-0.167,P?0.05),与面对无相关性(r= 0.078,P>0.05)。结论 癌症患者疾病进展恐惧处于中等水平,医护人员可根据癌症患者疾病进展恐惧程度及其影响因素为干预的实施拓展思路,以减轻患者恐惧心理,提高疗效及生活质量。

Current status of fear of progression in cancer patients and its influencing factors.

【Abstract】 Objective To investigate the current status of fear of progression (FoP) among cancer patients, toanalyze its influencing factors, and to provide references for the improvement of personalized nursing care for clinicalcancer patients. Methods Using the convenient sampling method, 152 cancer patients who were treated in the Depart-ment of oncology in the First Affiliated Hospital of Guangxi Medical University from August 2017 to February 2018were investigated. The survey was conducted by using self-designed general information questionnaire, fear of progres-sion questionnaire-short form (FoP-Q-SF) and medical coping modes questionnaire (MCMQ). Multivariate stepwise re-gression model was used to analyze the related factors influencing the FoP in cancer patients. Results The score of Fopin all the 152 cancer patients was (30.07±9.68), among which there were 43 patients (28.3%) with mild FoP, 64 patients(42.1%) with moderate FoP and 45 patients (29.6%) with severe FoP. Univariate analysis showed that the difference wasstatistically significant (P<0.05) in patients with different gender, age, place of residence, type of medical insurance andcurrent working status. Multivariate stepwise regression analysis showed that the gender, age, place of residence andmedical insurance type were the factors influencing cancer patients' fear of progression (P?0.05). The scores of MC-MQ of cancer patients were confrontation (18.26±1.17), avoidance (14.80±1.76) and surrender (11.98±1.13). Therewas a significant positive correlation between FoP and avoidance (r=0.223, P?0.01), significant negative correlationwith surrender (r=-0.167, P?0.05), and no correlation with confrontation (r=0.078, P>0.05). Conclusion Cancerpatients' FoP is at a moderate level. Health care personnel can develop targeted interventions according to the cancerpatients' FoP and their influencing factors, so as to reduce the fear of patients, improve treatment effect and their quali-ty of life.
