标题:先天性心脏病伴肺动脉高压患者血清BNP、VEGF及TGF-β1水平的表达及临床意义

【摘要】 目的 探讨先天性心脏病伴肺动脉高压患者血清B型尿钠肽(BNP)、血管内皮生长因子(VEGF)及转化生长因子-β1 (TGF-β1)水平的表达及其临床意义。方法 选择2014年1月至2017年6月深圳市人民医院收治的左向右分流型先天性心脏病患者86例作为研究组,按照肺动脉收缩压(PASP)分为A组(PASP≤30 mmHg,n=20)、B组(30 mmHg≤PASP<50 mmHg,n=42)和C组(PASP≥50 mmHg,n=24),另选取同期医院体检的健康人80例作为对照组。比较研究组和对照组、不同PASP分组患者的血清BNP、VEGF及TGF-β1水平,并采用Pearson相关性分析分析其与 PASP的相关性。结果 研究组患者的血清 BNP、VEGF及 TGF-β 1水平分别为(236.76±75.43) pg/mL、(403.32±89.87) pg/mL、(837.53±102.25) pg/mL,均明显高于对照组的(88.36±26.73) pg/mL、(86.73±22.45) pg/mL、(128.81±32.13) pg/mL,差异均有统计学意义(P<0.05);患者血清BNP、VEGF及TGF-β1水平随PASP升高而升高,不同PASP分组患者血清BNP、VEGF及TGF-β1水平两两比较差异均有统计学意义(P<0.05);Pearson相关性分析结果显示,先天性心脏病伴肺动脉高压患者血清BNP、VEGF及TGF-β1水平与PASP呈正相关(r=0.684、0.572、0.521,P<0.05),血清BNP、VEGF与TGF-β1呈正相关(r=0.512、0.554,P<0.05)。结论 左向右分流型先天性心脏病存在血清BNP、VEGF及TGF-β1异常升高,三者与PASP呈正相关。

【Abstract】 Objective To investigate the expression serum type B natriuretic peptide (BNP), vascular endotheli-al growth factor (VEGF) and transforming growth factor-β 1 (TGF-β 1) levels in patients with congenital heart diseasecomplicated with pulmonary hypertension and its clinical significance. Methods A total of 86 patients with left to rightshunt congenital heart disease, who were treated in Shenzhen People's Hospital from January 2014 to June 2017, were se-lected as the study group and according to the pulmonary artery systolic pressure (PASP) divided into the group A (PASP<30 mmHg, n=20), group B (30 mmHg≤PASP<50 mmHg, n=42), group C (PASP≥50 mmHg, n=24). Another 80 healthypeople in the same period were selected as the control group. The levels of serum BNP, VEGF and TGF-β 1 between thestudy group and the control group and different PASP groups were compared, and the correlation between them and PASPwas analyzed. Results The serum levels of BNP, VEGF and TGF-β 1 in the study group were (236.76±75.43) pg/mL,(403.32 ± 89.87) pg/mL, (837.53 ± 102.25) pg/mL, respectively, which were significantly higher than corresponding(88.36±26.73) pg/mL, (86.73±22.45) pg/mL, (128.81±32.13) pg/mL in the control group (P<0.05). With the increase ofPASP, the serum levels of BNP, VEGF and TGF-β 1 increased, and the levels of serum BNP, VEGF and TGF-β 1 in pa-tients with different PASP groups were statistically different (P<0.05). Pearson correlation analysis showed that the lev-els of serum BNP, VEGF and TGF-β 1 were positively correlated with PASP in patients with congenital heart diseasecomplicated with pulmonary hypertension (r=0.684, 0.572, 0.521, all P<0.05), and the serum BNP, VEGF were positive-ly correlated with TGF-β1 in patients with congenital heart disease complicated with pulmonary hypertension (r=0.512,0.554, all P<0.05). Conclusion The serum levels of BNP, VEGF and TGF-β1 are increased in left to right shunt con-genital heart disease, and the three of them are positively correlated with PASP.
