      标题:低剂量螺旋CT扫描在早期肺癌诊断中的应用价值

      【摘要】 目的 探讨低剂量螺旋CT扫描在早期肺癌诊断中的应用价值。方法 选取2014年12月至2016年10月广汉市人民医院呼吸内科收治的56例早期肺癌患者作为研究对象,先进行低剂量CT扫描检查筛查,3 d后再实施常规剂量螺旋CT扫描。比较两种扫描方式的肺癌检出率及对结节分叶、毛刺、空洞、钙化的显示情况。同时,观察两种扫描方式的图像质量及有效放射剂量。结果 低剂量CT对早期肺癌检出率为91.1%,与常规剂量CT的96.4%比较差异无统计学意义(P>0.05);低剂量CT与常规剂量CT对结节分叶(23.21% vs 25.00%)、毛刺(28.57%vs 30.36%)、空洞(14.29% vs 16.07%)及钙化(23.21% vs 25.00%)的显示情况比较差异亦均无统计学意义(P>0.05);低剂量CT图像质量优良率为 91.07%,稍低于常规剂量CT的 100.00%,但差异无统计学意义(P<0.05);低剂量CT有效放射剂量为(0.66±0.02) mGy·cm,明显低于常规剂量 CT的(0.89±0.03) mGy·cm,差异有统计学意义(P<0.05)。结论 低剂量螺旋CT检查能在显著降低辐射量的同时保证早期肺癌检出率,值得临床推广。

Value of low-dose spiral CT scanning in the diagnosis of early lung cancer.

【Abstract】 Objective To investigate the value of low-dose spiral CT scanning in the diagnosis of early lungcancer. Methods From December 2014 to October 2016, 56 patients of early lung cancer in Guanghan People's Hospi-tal were selected as the research subjects. Low-dose spiral CT scan was performed first, and routine CT scan was per-formed 3 d later. The detection rates of lung cancer in two kinds of scanning methods were compared, as well as the dis-play of the nodule, burr, cavity and calcification. At the same time, the image quality and effective radiation dose of twoscanning methods were observed. Results There was no significant difference in the detection rate of early lung cancerbetween low-dose spiral CT and the routine CT (91.1% vs 96.4%, P>0.05). There was also no significant difference be-tween the two groups in the display of leaf (23.21% vs 25.00%), burr (28.57% vs 30.36%), cavity (14.29% vs 16.07%)and calcification (23.21% vs 25.00%) of nodules (P>0.05). The quality of low-dose scanning images was slightly lowerthan that of conventional dose (91.07% vs 100.00%), but the difference was not statistically significant (P>0.05). The ef-fective dose of low-dose scanning was (0.66±0.02) mGy·cm, significantly lower than (0.89±0.03) mGy·cm of conven-tional dose scanning (P<0.05). Conclusion Low-dose spiral CT scan is helpful to reduce the radiation dose and de-crease the detection rate of early lung cancer, which is worthy of clinical promotion.
