标题：血清 IgE、IgG在不同程度慢性荨麻疹中的表达及意义

      【摘要】 目的 探讨不同程度慢性荨麻疹(CU)患者血清 IgE、IgG的表达水平及其临床意义。方法 选取深圳市松岗人民医院皮肤科2010年3月至2015年12月收治的90例CU患者为研究对象，根据荨麻疹活动评分（UAS评分）分为轻度组(n=36例)、中度组(n=30例)、重度组(n=24例)，比较各组间血清总 IgE、IgG水平的差异。结果 随着慢性荨麻疹病情的加重，三组患者的UAS评分分别为(1.96±0.64)分、(3.93±1.95)分和(5.95±2.64)分，差异有统计学意义(P<0.05)，而血清总 IgE、IgG水平则呈逐渐递增趋势，但组间比较差异均无统计学意义(P>0.05)；以总 IgE≥175 IU/mL或 IgG≥65 IU/mL为分界值时，轻度组、中度组和重度组患者的血清总 IgE [(188.59±65.33) IU/mL vs (247.46±65.31) IU/mL vs (298.57±59.42) IU/mL]、IgG [(69.83±18.33) IU/mL vs (79.29±18.31) IU/mL vs (89.18±19.43) IU/mL]水平比较差异均有统计学意义(P<0.05)，而总 IgE<175 IU/mL或 IgG<65 IU/mL时，三组间血清 IgE、IgG水平比较差异均无统计学意义(P>0.05)。结论 慢性荨麻疹患者的血清总 IgE水平过高，其不仅与 IgE介导的Ⅰ型变态反应有关，还与IgG引起的某些食入性过敏原有一定相关性，其检测可了解慢性荨麻疹病程及严重程度，为患者诊治提供帮助。

【Abstract】 Objective To investigate the expression of IgE and IgG in serum of patients with chronic urticaria(CU) in different degrees and their clinical significance. Methods From March 2010 to December 2015, 90 patients ofCU selected in Shenzhen Songgang People's Hospital, according to urticaria activity score (UAS score), were divided in-to mild group (n=36), moderate group (n=30), severe group (n=24). The total serum IgE and IgG levels in the threegroups were analyzed. Results In the mild, moderate, and severe groups, the UAS scores were (1.96±0.64), (3.93±1.95), (5.95±2.64) respectively(P<0.05), and the total serum IgE, IgG levels showed a gradual increasing trend but withno significant difference (P>0.05). With IgE≥175 IU/mL or IgG≥65 IU/mL, the differences in total serum IgE (188.59±65.33) IU/mL vs (247.46±65.31) IU/mL vs (298.57±59.42) IU/mL and serum IgG levels (69.83±18.33) IU/mL vs (79.29±18.31) IU/mL vs (89.18±19.43) IU/mL between the three groups were statistically significant (P<0.05). With total IgE<175 IU/mL or IgG<65 IU/mL, there was no significant difference in the level of serum IgE and IgG between the threegroups (P>0.05). Conclusion Serum total IgE level in CU patients is high, which is not only related to IgE-mediatedtypeⅠ allergy, but also has a correlation with IgG-caused food allergens. Its detection can understand the course and se-verity of chronic CU, and provide help for the diagnosis and treatment of patients.
