      标题:缺氧缺血性脑病新生儿免疫功能的变化及其影响因素

      【摘要】 目的 探讨缺氧缺血性脑病(HIE)新生儿免疫功能的变化及其影响因素。方法 选择2014年5月至2016年10月期间深圳市龙岗区龙岗中心医院新生儿科收治的32例HIE患儿为研究对象(HIE组),选择同期30例健康新生儿纳入对照组,比较两组新生儿血免疫球蛋白(IgG、IgM、IgA)水平,并分析影响免疫功能的相关因素。结果 HIE组患儿的血IgM、IgA水平分别为(0.14±0.04) g/L、(0.13±0.03) g/L,明显低于对照组的(0.17±0.03) g/L、(0.18±0.04) g/L,差异均有统计学意义(P<0.05);两组患儿的血 IgG比较差异无统计学意义(P>0.05);多元线性回归分析结果显示,HIE患儿血 IgG、IgM水平与羊水污染呈负相关性(P<0.05),IgA与Apgar评分、母乳喂养呈正相关性(P<0.05)。结论 HIE患儿可出现体液免疫功能紊乱,喂养方式、羊水污染程度、Apgar评分对体液免疫功能有一定影响。

【Abstract】 Objective To investigate the changes of immune function in neonates with hypoxic ischemic en-cephalopathy (HIE) and its influencing factors. Methods From May 2014 to October 2016, 32 HIE neonates in Depart-ment of Neonatal Pediatrics, Longgang Central Hospital of Longgang District of Shenzhen were enrolled in the HIEgroup, and 30 healthy neonates were included into the control group. Blood immunoglobulin (IgG, IgM, IgA) levels werecompared between the two groups, and the related factors that affect immune function were analyzed. Results The levels ofserum IgM and IgA in HIE group were (0.14±0.04) g/L, (0.13±0.03) g/L, significantly lower than (0.17±0.03) g/L, (0.18±0.04) g/L in the control group (P<0.05). The serum IgG in the two group was of no statistically significant difference (P>0.05). Multivariate linear regression analysis showed that the IgG and IgM was negatively correlated with amniotic fluidpollution, and IgA was positively correlated with Apgar score and breast feeding (P<0.05). Conclusion HIE can inducechanges of humoral immune function, and the feeding mode, the degree of amniotic fluid pollution, and the Apgar scorehave some influence on the humoral immune function.
