标题:冠心病患者左室舒张末期压力与冠心病严重程度的相关性研究

【摘要】 目的 探讨冠心病患者左室舒张末期压力与冠心病和冠状动脉病变程度之间的关系。方法 选取2014年3月至2017年3月期间在广东省农垦中心医院同时行冠状动脉造影及心导管检查患者613例,根据造影结果将患者分为冠状动脉病变(CHD)组490例与非CHD组123例,利用Gensini评分及病变的血管数目评估冠状动脉的病变严重程度,利用心导管检查测量并记录左室舒张末期压力(LVEDP)值,分析LVEDP与CHD之间的关系及LVEDP与冠脉严重程度之间的关系。结果 CHD组患者LVEDP为(11.03±5.12) mmHg,明显高于CHD组的(9.34±4.89) mmHg,差异有统计学意义(P<0.05);LVEDP与CHD独立相关(OR=1.11,95%CI: 1.02~1.29,P=0.02),LVEDP随着病变血管数目的增加而增加(P<0.05);多因素线性分析结果显示LVEDP与Gensini评分相关(β=0.034,P=0.00)。结论 LVEDP与冠心病有关,且随冠脉病变范围和程度的增加而增加。

【Abstract】 Objective To investigate the relationship between left ventricular end-diastolic pressure (LVEDP)and coronary artery disease (CHD) and severity of coronary artery disease in patients with CHD. Methods A total of613 patients with CHD who underwent coronary angiography and cardiac catheterization in Guangdong AgribusinessCentral Hospital from March 2014 to March 2017 were selected. The patients were divided into CHD group (n=490) andnon-CHD group (n=123) by the result of coronarography. The severity of coronary artery disease was evaluated by thenumber of stenotic vessles and Gensini score. The value of LVEDP was recorded and measured by cardiac catheteriza-tion. The relationship between LVEDP and CHD as well as its severity was analyzed. Results The value of LVEDPwas (11.03±5.12) mmHg in CHD group, which was significantly higher than (9.34±4.89) mmHg in non-CHD group (P<0.05). The results showed that there was an independent correlation between LVEDP and CHD (OR=1.11, 95% CI:1.02-1.29, P=0.02), and the LVEDP increased with the increase of the number of diseased vessels (P<0.05). Linear re-gression analysis showed that LVEDP was correlated with Gensini score (β=0.034, P=0.00). Conclusion LVEDP is as-sociated with CHD, which was increased with the increase of the range and degree of coronary artery disease.
