标题：MAIT细胞恒定链Jα33在 IgA肾病患者中的表达及意义

      【摘要】 目的 探讨黏膜相关恒定链T淋巴细胞(MAIT)恒定链 Jα33在 IgA肾病(IgAN)患者中的表达及其临床意义。方法 收集2014年1月1日至2015年12月31日宜昌市中心人民医院住院患者肾脏标本共55例，其中未经免疫治疗的不同程度蛋白尿 IgA肾病患者肾穿标本30例，经治疗有效和无效的 IgA肾病患者肾穿标本各10例，选择因车祸引起腹部复合性钝性损伤并发有肾脏损伤需进行肾脏部分切除的住院患者肾脏标本 5例作为对照组，分别检测MAIT细胞恒定链 Jα33在患者外周血及肾脏组织中的表达。结果 与对照者比较，MAIT细胞恒定链 Jα33 mRNA及蛋白水平在 IgAN患者肾组织中明显上调，差异具有统计学意义(P<0.05)，而在外周血中的表达比较差异无统计学意义(P>0.05)；MAIT细胞恒定链 Jα33在不同程度蛋白尿 IgAN患者肾脏组织中的表达差异无统计学意义(P>0.05)；与未经免疫治疗的 IgAN患者比较，MAIT细胞恒定链 Jα33在治疗有效的 IgAN患者肾脏组织中的表达水平明显下调，差异具有统计学意义(P<0.05)，而与治疗无效组的表达比较差异无统计学意义(P>0.05)。结论 IgAN患者肾组织MAIT细胞恒定链 Jα33表达量增多，经治疗有效后MAIT细胞恒定链 Jα33表达减少；Jα33+MAIT可能参与 IgAN发病过程。

【Abstract】 Objective To investigate the expression and clinical significance of mucosal-associated invariant T(MAIT) Jα33 in IgA nephropathy (IgAN) patients. Methods A total of 55 renal specimens from the Central People'sHospital of Yichang from January 1, 2014 to December 31, 2015 were collected, including 30 specimens from IgA ne-phropathy patients with varying degrees of proteinuria without immunotherapy, 10 from IgA nephropathy patients treat-ed effectively, 10 from IgA nephropathy patients treated ineffectively, 5 from inpatients with blunt abdominal injury com-plicating renal injury (caused by traffic accident) scheduled for partial nephrectomy (control group). The expression ofMAIT cell Jα 33 was detected in peripheral blood and kidney tissues respectively. Results Compared with controlgroup, the levels of Jα33 mRNA and protein in renal tissue of IgAN patients was significantly increased (P<0.05), whilethere was no significant difference in the peripheral blood (P>0.05). There was no significant difference in Jα33 expres-sion between different degrees of proteinuria in kidney tissues of IgAN patients (P>0.05). Compared with the untreatedIgAN patients, the expression level of Jα33 in renal tissue of patients treated effectively was significantly decreased (P<0.05), while the levels in the patients treated ineffectively was similar (P>0.05). Conclusion The expression level ofMAIT cell Jα33 is increased in IgAN patients, and the level reduced after effective treatment. MAIT cell Jα33 may par-ticipate in the pathogenesis of IgAN.
