      标题:急性心肌梗死患者血清心肌酶、心肌蛋白与幽门螺杆菌感染的关联性

      【摘要】 目的 了解急性心肌梗死(AMI)患者血清心肌酶、心肌蛋白和幽门螺杆菌感染情况,并探讨其关联性。方法 2015年1月至2016年12月随机抽取于十堰市太和医院治疗的确诊急性心肌梗死患者100例和体检中心健康体检的正常人50例,测定所有受检者血清心肌激酶(CK)、心肌激酶同工酶B (CK-MB)、肌钙蛋白T (cTnT)、肌钙蛋白 I (cTnI),并检测幽门螺杆菌(Hp)感染情况,分析心肌酶、心肌蛋白与幽门螺杆菌感染的关联性。结果 (1)AMI组CK、CK-MB、cTnT和 cTnI分别为(182.6±28.2) IU/L、(36.7±6.3) IU/L、(0.91±0.22) μg/L和(1.66±0.31) μg/L,而对照组分别为(34.7±5.2) IU/L、(7.0±1.6) IU/L、(0.11±0.06) μg/L和(0.38±0.10) μg/L,组间比较差异均有统计学意义(P<0.05);(2)AMI组患者Hp总感染率为79%,明显高于对照组的52%,差异有统计学意义 (P<0.05);(3)AMI组患者的CK-MB、cTnT和 cTnI与Hp感染存在关联,其关联系数 r分别为0.343、0.458和0.320 (P<0.05)。结论 血清心肌酶和心肌蛋白可作为AMI诊断、病情判断和预后预测的重要指标,AMI患者Hp感染发生率高,Hp感染与CK-MB、cTnT和cTnI有关联,且可能是AMI的危险因素。

Associations between serum myocardial enzymes, troponin and Helicobacter pylori infection in acute myocardialinfarction patients.

【Abstract】 Objective To study the status myocardial enzymes, troponin and Helicobacter pylori (Hp) infectionin acute myocardial infarction (AMI) patients, and to analyze their association. Methods From January 2015 to Decem-ber 2016, 100 AMI confirmed patients and 50 healthy individuals were selected randomly from Taihe Hospital of Shi-yan, Hubei University of Medicine, to detect serum creatine Kinase (CK), creatine kinase isoenzyme MB (CK-MB), car-diac troponin T (cTnT), cardiac troponin I (cTnI) and Hp infection. The relationships between myocardial enzymes, tro-ponin and Hp infection were analyzed. Results (1) CK, CK-MB, cTnT and cTnI levels were (182.6±28.2) IU/L, (36.7±6.3) IU/L, (0.91±0.22) μg/L and (1.66±0.31) μg/L in AMI group, respectively, versus (34.7±5.2) IU/L, (7.0±1.6) IU/L,(0.11±0.06) μg/L and (0.38±0.10) μg/L in the control group (P<0.05). (2) The Hp infection rate was 79% in AMI groupversus 52% in control group (P<0.05). (3) CK-MB, cTnT and cTnI had associations with Hp infection in AMI group,with correlation coefficient 0.343, 0.458 and 0.320 respectively (P<0.05). Conclusion Serum myocardial enzymes andtroponin can be considered as important reference indexes for AMI diagnosis, illness condition and prognosis evaluation.Hp infection rate is high in AMI patients, and Hp infection is a possible risk factor of AMI and associated with CK-MB,cTnT and cTnI.
