标题：早期集束化护理对急性腺病毒肺炎合并脓毒症患儿肠功能的影响

      【摘要】 目的 探讨早期集束化护理对急性腺病毒(ADV)肺炎合并脓毒症患儿肠功能的影响。方法 选取广东科技大学附属三水医院 2015年 6月至 2016年 11月儿科收治的ADV感染合并脓毒症患儿 80例为研究对象，根据随机数表法分为观察组和对照组，各 40例，对照组针对脓毒症并发症进行护理，观察组则早期应用集束化护理干预方案，干预疗程均为 7 d。比较两组患者干预前后的肠功能障碍评分、急性生理学与慢性健康状况Ⅱ(APACHEⅡ)评分以及血清超敏C反应蛋白(hs-CRP)的变化。结果 干预后，观察组肠道功能障碍评分为(1.36±0.58)分，明显低于对照组的(1.92±0.52)分，差异有统计学意义(P<0.05)；观察组血清 hs-CRP及APACHEⅡ评分分别为(14.27±2.55) mg/L和(11.72±2.35)分，明显低于对照组的(17.34±3.08) mg/L和(13.93±2.44)分，差异均有统计学意义(P<0.05)。结论 对腺病毒感染合并脓毒症患儿进行早期集束化护理干预可有效预防肠功能障碍的发生、进而减轻炎症反应，促进疾病的恢复。

【Abstract】 Objective To investigate the effect of early cluster nursing on intestinal function in children withacute adenovirus (ADV) pneumonia complicated by sepsis. Methods A total of 80 children with acute ADV pneumo-nia complicated by sepsis, who admitted to Department of Pediatrics in our hospital from June 2015 to November 2016,were selected and divided into the observation group (n=40) and the control group (n=40) according to the random num-ber table method. The control group was given nursing care of complications and sepsis, and the observation group wastreated with early application of cluster nursing interventions. All the patients were treated for 7d. The changes of the in-testinal dysfunction score, APACHEⅡ score and hs-CRP of the two groups before and after the intervention were com-pared. Results After the intervention, the intestinal dysfunction score of the observation group was (1.36±0.58), whichwas significantly lower than (1.92±0.52) of the control group (P<0.05); the serum hs-CRP and APACHE score in the ob-servation group was (14.27±2.55) mg/L, (11.72±2.35), respectively, significantly lower than (17.34±3.08) mg/L, (13.93±2.44) in the control group (P<0.05). Conclusion Early cluster nursing intervention can effectively prevent the occur-rence of intestinal dysfunction in children with adenovirus infection complicated by sepsis, then reduce the inflammatoryresponse and promote the recovery of the patients from the disease.
