标题：维持性血液透析患者胃黏膜病变与血清胃蛋白酶原水平变化

      【摘要】 目的 探讨维持性血液透析(MHD)患者胃黏膜病变及血清胃蛋白酶原(PG)水平的变化情况。方法 选择 2015年 1月至 2016年 12月期间我院收治的 62例MHD患者纳入观察组，随机选择同期 62例健康体检者纳入对照组，比较两组受检者的血清PGⅠ、PGⅡ和PGⅠ/PGⅡ水平；对观察组患者进行胃镜检查以观察胃黏膜病变情况。结果 观察组患者最常见的胃黏膜病变为慢性糜烂性胃炎(35.48%)、胃十二指肠溃疡(32.26%)及慢性出血性胃炎(25.81%)；观察组患者的PGⅠ和PGⅠ/PGⅡ比值均分别为(212.4±85.6) ?g/L和(20.8±5.6)，明显高于对照组的(106.3±43.2) ?g/L和(11.4±3.9)，差异均有统计学意义(P<0.05)。结论 慢性糜烂性胃炎、胃十二指肠溃疡及慢性出血性胃炎是维持性血液透析患者常见胃黏膜病变，且患者常伴有PGⅠ和PGⅠ/PGⅡ比值的增高。

【Abstract】 Objective To investigate the gastric mucosal lesions and serum pepsinogen (PG) changes in main-tenance hemodialysis (MHD) patients. Methods A total of 62 cases of MHD patients, who admitted to our hospitalfrom January 2015 to December 2016, were included into the observation group. Meanwhile, 62 cases of healthy peoplewere randomly selected as the control group. The levels of serum PGⅠ, and PG Ⅱ, PG Ⅰ/PG Ⅱ of the two groupswere compared; the patients in the observation group were examined by gastroscopy to observe the pathological changesof gastric mucosa. Results The most common gastric mucosal lesions in MHD patients was chronic erosive gastritis(35.48%), gastric ulcer (32.36%) and chronic hemorrhagic gastritis (25.81%). The PGⅠ and PGⅠ/PGⅡ in the observa-tion group were respectively (212.4±85.6) μg/L and (20.8±5.6), which were significantly higher than (106.3±43.2) μg/Land (11.4 ± 3.9) in the control group (P<0.05). Conclusion The chronic erosive gastritis, gastroduodenal ulcer andchronic hemorrhagic gastritis are common gastrointestinal lesions in patients with maintenance hemodialysis, and the pa-tients are often accompanied by the increases of PGⅠ and PGⅠ/PGⅡ.
