标题：高龄孕妇妊娠结局及其影响因素分析

      【摘要】 目的 比较高龄孕妇与非高龄孕妇妊娠结局的差异，并探讨影响妊娠结局的相关因素。方法 回顾性分析达州市达县人民医院2013年4月至2016年4月期间收治的1 035例孕妇的相关临床资料，其中高龄孕妇组501例，非高龄孕妇组534例，比较两组孕妇的妊娠合并症、并发症及妊娠结局，并采用χ2检验分析相关因素对其的影响。结果 妊娠期高血压、妊娠期肝内胆汁淤积症(ICP)、妊娠期糖尿病，流产率、新生儿畸形、胎膜早破等方面，高龄组孕妇的发生率均明显高于非高龄组，差异均具有统计学意义(P<0.05)；高龄组孕妇的自然分娩率明显低于非高龄组，而难产率和死胎率却明显高于非高龄组，差异均具有统计学意义(P<0.05)；高龄组孕妇的多次流产、心理忧郁或焦虑、不良生育史、疤痕子宫、生殖功能评价差、肥胖等6方面的发生率均明显高于非高龄组，差异均具有统计学意义(P<0.05)。结论 高龄孕妇的妊娠结局较差，多次流产、心理忧郁或焦虑、不良生育史、疤痕子宫、生殖功能评价差、肥胖是影响高龄孕妇妊娠结局的因素。

Analysis of pregnancy outcome and its influencing factors in elderly pregnant women.

【Abstract】 Objective To compare difference of pregnancy outcomes between elderly and non-elderly pregnantwomen, and to explore the related factors affecting the outcome of pregnancy. Methods The clinical data of 1 035 preg-nant woman in Daxian People's Hospital from April 2013 to April 2016 were retrospectively analyzed. Pregnancy com-plications, complications and pregnancy outcomes were compared between the two groups. Chi-square test was used toanalyze the influence of related factors. Results The incidence rate of pregnancy induced hypertension, intrahepaticcholestasis of pregnancy (ICP), gestational diabetes mellitus, abortion rate, neonatal malformation and premature ruptureof membranes of elderly pregnant women group were significantly higher than those of the non-elderly pregnant womengroup (P<0.05). The natural labor rate in elderly pregnant women group was significantly lower than that in non-elderlypregnant women group, but the dystocia and stillbirth rate were significantly higher than those of the non-elderly preg-nant women group (P<0.05). The incidence rate of multiple abortion, psychological depression or anxiety, abnormalchild-bearing history, scar uterus, poor reproductive function evaluation and obesity in elderly pregnant women groupwere significantly higher than those of the non-elderly pregnant women group (P<0.05). Conclusion Poor pregnancyoutcome, multiple miscarriages, psychological depression or anxiety, abnormal child-bearing history, scar uterus, poor re-productive function evaluation, obesity are the important factors affecting the outcome for elderly pregnant women.
