      标题:男性血清尿酸水平与血脂及其部分比值的相关性

      【摘要】 目的 研究成年男性血清尿酸(UA)水平与血脂各组分及部分比值的相关性。方法 纳入武汉大学人民医院 2016年 4~8月男性体检者 1 164例,按年龄、尿酸水平分组,其中 652例为随机样本,512例为健康对照组,检测各组血清UA、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、小而密低密度脂蛋白胆固醇(sdLDL-C)水平,并计算 sdLDL-C/HDL-C、TC/HDL-C值,进行统计学分析。结果 男性血清UA、sdLDL-C水平及 sdLDL-C/HDL-C与 TC、TG、HDL-C、LDL-C、sdLDL-C、TC/HDL-C在置信度(双侧)为 0.01时均显著相关;成年男性血清UA水平分布:20~40岁为 361.00 ?mol/L (330.75~394.00 ?mol/L);41~60岁为355.00 ?mol/L (319.00~395.00 ?mol/L);61~80岁为367.00 ?mol/L (323.00~406.00 ?mol/L);高UA组血清各项指标分布:TC为 4.670 0 mmol/L (4.210 0~5.370 0 mmol/L),TG为 1.680 0 mmol/L (1.240 0~2.480 0 mmol/L),HDL-C为 1.060 0 mmol/L (0.920 0~1.220 0 mmol/L),LDL-C为 2.670 0 mmol/L (2.230 0~3.120 0 mmol/L),sdLDL-C为 0.970 0 mmol/L (0.690 0~1.290 0 mmol/L),TC/HDL-C为 4.453 7 (3.795 8~5.061 7),sdLDL-C/HDL-C为 0.905 8(0.643 8~1.211 3);高 UA组与正常对照组比较,血清 TC、TG、HDL-C、LDL-C、sdLDL-C、sdLDL-C/HDL-C、TC/HDL-C差异均有统计学意义(P<0.05);男性20~40岁人群血清UA、HDL-C、TG、sdLDL-C水平与正常对照组比较差异有统计学意义(P<0.05),血清TC、LDL-C水平差异无统计学意义(P>0.05)。结论 成年男性血清尿酸水平与血脂水平密切相关;20~40岁男性总体血清尿酸及血脂水平已显著偏离正常水平;sdLDL-C/HDL-C对评价体内血脂代谢情况有一定的临床价值。

Correlation between male serum uric acid and serum lipids and its associated ratios.

【Abstract】 Objective To study the correlation between serum uric acid (UA) levels and serum lipid fractions and itsratios in adult males. Methods Collection of 1 164 male cases from the MEC in Renmin hospital of Wuhan University weregrouped by age and UA level. Among them, 652 cases were random samples, and the other 512 cases were healthy control. Se-rum UA, total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cho-lesterol (LDL-C), and small dense low-density lipoprotein cholesterol (sdLDL-C) levels were detected in each group. The ratioof sdLDL-C/HDL-C, TC/HDL-C were calculated and compared by statistical analysis. Results UA, sdLDL-C levels,sdLDL-C/HDL-C and TC, TG, HDL-C, LDL-C, sdLDL-C, TC/HDL-C in the serum of male patients were significantly cor-related, since the confidence level (bilateral) was 0.01. Distribution of serum UA levels in adult males were 361.00 μmol/L(330.75-394.00 μmol/L) for the 20-40-year olds, 355.00 μmol/L (319.00-395.00 μmol/L) for the 41-60-year olds, 367.00 μmol/L(323.00-406.00 μmol/L) for the 61~80-year olds. Distribution of serum index in high UA(HUA) group were TC 4.670 0 mmol/L (4.2100-5.370 0 mmol/L), TG 1.680 0 mmol/L (1.240 0-2.480 0 mmol/L), HDL-C 1.060 0 mmol/L (0.920 0-1.220 0 mmol/L),LDL-C 2.6700 mmol/L (2.230 0-3.120 0 mmol/L), sdLDL-C 0.970 0 mmol/L (0.690 0-1.290 0 mmol/L), TC/HDL-C4.453 7 (3.795 8-5.061 7), sdLDL-C/HDL-C 0.9058 (0.643 8-1.211 3). The serum TC, TG, HDL-C, LDL-C, sdLDL-C,sdLDL-C/HDL-C and TC/HDL-C were different between the HUA group and the control group, and the difference was statisticallysignificant (P<0.05). The serum levels of UA, HDL-C, TG and sdLDL-C in 20-40-year-old males were statistically signifi-cant different from those of the control group (P<0.05); while there was no no statistically significant difference in serum levelsof TC and LDL-C (P>0.05). Conclusion There is a closely relationship between serum uric acid level and lipid level in adultmales. The serum levels of UA and blood lipid in 20-40-year-old men had significantly deviated from the normal level. Theratio of sdLDL-C/HDL-C has some clinical value in the evaluation of lipid metabolism in vivo.
