      标题:CK5/6、EGFR、p53与Ki67在乳腺癌组织中的表达及其意义

      【摘要】 目的 检测表皮生长因子受体(EGFR)、细胞角蛋白 5/6 (CK5/6)、p53蛋白(p53)和细胞增殖核抗原Ki67 (Ki67)在乳腺癌中的表达及其表达的相关性,并探讨其与乳腺癌患者的临床和病理特征的关系。方法 回顾性分析东莞市第三人民医院2010年1月至2011年12月间108例经病理确诊的乳腺癌患者的临床及病理资料。免疫组化检测EGFR、CK5/6、p53和Ki67在乳腺癌组织中的表达,Spearman相关分析蛋白表达的相关性,χ2检验或Fisher精确检验分析蛋白表达与患者临床及病理指标之间的关系。结果 EGFR、CK5/6、p53和Ki67在乳腺癌组织中阳性表达率分别为 17.6% (19/108)、25.9% (28/108)、42.6% (46/108)和 51.9% (56/108);EGFR与CK5/6和 p53呈正相关(r=0.282,r=0.241),CK5/6与 p53和Ki67呈正相关(r=0.217,r=0.190),p53和Ki67呈正相关(r=0.193),相关性均有统计学意义(P<0.05);EGFR、CK5/6、p53和Ki67在三阴乳腺癌组织中的表达比非三阴乳腺癌组织中的表达更高,差异均有统计学意义(P<0.05);与CK5/6或EGFR阴性的乳腺癌患者比较,CK5/6或EGFR阳性的乳腺癌患者的中位复发时间相对较短,差异均有统计学意义(P<0.05)。结论 EGFR和CK5/6在乳腺癌组织中的高表达提示CK5/6和EGFR可以作为乳腺癌预后不良因素。

Expression of CK5/6, EGFR, p53 and Ki67 in breast cancer and their significance.

【Abstract】 Objective To detect the expression of cktokeratin5/6 (CK5/6), epidermal expression of growth fac-tor receptor (EGFR), p53 and nuclcar-associated antigen Ki67 (Ki67) in breast cancer, and investigate the association oftheir expression with clinical and pathological features in breast cancer. Methods We performed a retrospective analy-sis of 108 breast cancer patients from Dongguan Third People's Hospital between January 2010 and December 2011. Ex-pression of EGFR, CK5/6, p53 and ki67 were detected in breast cancer tissues by immunohistochemical methods, andtheir expression correlation was analyzed by Spearman analysis. Chi-square or Fisher's exact test was used to investigatetheir relationships with clinical and pathological indexes. Results Out of the 108 cases, the positive rates for EGFR,CK5/6, p53 and Ki67 were 17.6% (19/108), 25.9% (28/108), 42.6% (46/108) and 51.9% (56/108), respectively. Correla-tion analysis indicated positive correlation between EGFR and CK5/6, p53 (r=0.282, r=0.241), positive correlation ofCK5/6 with p53 and Ki67 (r=0.217, r=0.190), and positive correlation between p53 and Ki67 (r=0.193) (P<0.05 for all).Expression of EGFR, CK5/6, p53和 Ki67 were found significantly up-regulated in triple negative breast cancer com-pared to non-triple negative breast cancer (P<0.05 for all). Median time to relapse was shorter for the patients with CK5/6 or EGFR-positive tumors compared to the ones with CK5/6 or EGFR-negative cases (P<0.05). Conclusion Theup-regulated expression of EGFR and CK5/6 in breast cancer may be correlated with a worse prognosis.
