      标题:社会支持对肺动脉高压患者生活质量的影响

      【摘要】 目的 了解肺动脉高压患者的生活质量,并探讨社会支持对肺动脉高压患者生活质量的影响。方法 采用整群抽样的方法选择2014年12月至2015年8月期间在解放军总医院海南分院住院治疗的84例肺动脉高压患者作为研究对象。分别采用简明健康调查量表(SF-36)和社会支持评定量表(SSRS)对患者的生活质量及社会支持进行问卷调查。结果 肺动脉高压患者生活质量各维度得分均低于常模,差异均有统计学意义(P<0.05);Pearson相关分析显示,肺动脉高压患者的社会支持总分与生活质量的生理功能(r=0.169,P<0.05)、生理职能(r=0.159,P<0.05)、躯体疼痛(r=0.239,P<0.05)、总体健康(r=0.225,P<0.05)、活力(r=0.247,P<0.05)、社会功能(r=0.223,P<0.05)、情感职能(r=0.279,P<0.05)及精神健康维度(r=0.509,P<0.05)得分均呈正相关。多元线性回归分析显示,影响肺动脉高压者生活质量的因素有:性别(β=-0.373,P<0.05)、6 min步行距离(β=-0.354,P<0.05)、是否合并右心衰竭(β=-0.109,P<0.05)及社会支持水平(β=0.233,P<0.05)。结论 肺动脉高压患者的生活质量不容乐观,今后应采取积极措施,特别是通过改善社会支持来提高患者的生活质量。

Effect of social support on quality of life among patients with pulmonary arterial hypertension.

【Abstract】 Objective To investigate the quality of life (QOL) in patients with pulmonary arterial hyperten-sion and to explore the effect of social support on quality of life among patients with pulmonary arterial hypertension.Methods A total of 84 patients with pulmonary arterial hypertension, who admitted to Hainan Branch of PLA GeneralHospital from December 2014 to August 2015, were selected as the research objects according to cluster sampling meth-od. QOL and social support were evaluated by the Medical Outcomes Study (MOS) 36-item short form health survey(SF-36) and the Social Support Rating Scale (SSRS), respectively. Results Compared with the general population, alldimension scores of SF-36 scale were significantly lower in patients with pulmonary arterial hypertension (P<0.05).Pearson correlation analysis showed that social support was significantly positively correlated with physical functioning(r=0.169, P<0.05), physical role (r=0.159, P<0.05), body pain (r=0.239, P<0.05), general health (r=0.225, P<0.05), vi-tality (r=0.247, P<0.05), social functioning (r=0.223, P<0.05), emotional role (r=0.279, P<0.05) and mental health (r=0.509, P<0.05) among patients with pulmonary arterial hypertension. The results of multiple linear regression analysisshowed that the associated factors of QOL were gender (β=-0.373, P<0.05), 6-minute walking distance (β=-0.354, P<0.05), right heart failure or not (β=-0.109, P<0.05) and social support (β=0.233, P<0.05). Conclusion The QOL of pa-tients with pulmonary arterial hypertension is not optimistic. In order to enhance the QOL of patients with pulmonary ar-terial hypertension, effective measures should be taken, especially measures about social support.
