      标题:血清胱抑素C、β2微球蛋白对早期糖尿病肾病的诊断价值

      【摘要】 目的 探讨血清胱抑素C(CysC)、β2微球蛋白(β2-MG)在早期2型糖尿病肾病(DN)中的诊断价值。方法 选取2016年10月至2017年10月间云浮市人民医院糖尿病患者140例,根据患者24 h尿微量白蛋白排泄率(UAER)将患者分为临床糖尿病肾病组29例、早期糖尿病肾病组42例和非糖尿病肾病组69例,同期健康体检者40例作为对照组,观察四组受检者CysC、β2-MG、尿素氮(BUN)的变化情况,并采用Pearson相关分析各指标间的相关性。结果 临床糖尿病肾病组患者的CysC、β2-MG和BUN水平明显高于其他三组,早期糖尿病肾病组患者的CysC、β2-MG和BUN水平又明显高于非糖尿病肾病组和对照组,差异均有统计学意义(P<0.05);临床糖尿病肾病组患者的CysC、β2-MG和BUN的阳性检出率明显高于其他三组,早期糖尿病组患者的CysC、β2-MG检出阳性率明显高于非糖尿病组和正常对照组,差异均有统计学意义(P<0.05);糖尿病肾病患者的CysC、β2-MG检出阳性率明显高于BUN,差异均有统计学意义(P<0.05);CysC与β2-MG、BUN呈正相关(r=0.652、0.534,P<0.05);同样β2-MG与CysC、BUN呈正相关(r=0.652、0.485,P<0.05)。结论 糖尿病肾病患者CysC、β2-MG水平均明显升高,且随着病变程度的加重呈升高趋势。在早期糖尿病肾病诊断中,CysC、β2-MG检出阳性率明显高于BUN,是一种良好的检测指标。

Value of serum cystatin C and β 2 microglobulin in the diagnosis of early diabetic nephropathy.

【Abstract】 Objective To investigate the diagnostic value of serum cystatin C (CysC) and β2 microglobulin(β2-MG) in early type 2 diabetic nephropathy. Methods A total of 140 patients with diabetes, who admitted to Yunfu Peo-ple's Hospital from October 2016 to October 2017, were selected and divided into the clinical diabetic nephropathy group,early diabetic nephropathy group and non-diabetic nephropathy group according to the 24-hour UAER. In the same peri-od, 40 cases of healthy physical examination were enrolled as the control group. The changes of CysC, β 2-MGM, bloodurea nitrogen (BUN) were observed, and the correlation between the four groups were observed. Results CysC, β2-MG,BUN in the clinical diabetic nephropathy group were significantly higher than those of the other three groups, and theCysC, β 2-MG, BUN in the early diabetic nephropathy group were significantly higher than those in the non-diabetic ne-phropathy group and the control group (P<0.05). The positive rate of CysC, β2-MG, BUN in the clinical diabetic nephropa-thy group was significantly higher than that of the other three groups, and the positive rate of CysC, β2-MG in the early di-abetic group was significantly higher than that of non-diabetic nephropathy group and control group (P<0.05). CysC waspositively correlated with β 2-MG and BUN (r=0.652, 0.534, P<0.05); β 2-MG was positively correlated with CysC andBUN (r=0.652, 0.485, P<0.05). Conclusion The levels of CysC and β2-MG are significantly increased in patients withdiabetic nephropathy, and they increase with the severity of the disease. In the early diagnosis of diabetic nephropathy,the positive rate of CysC and β2-MG is significantly higher than that of BUN, which is a good detection index.
