      标题:红细胞分布宽度与肝硬化Child-Pugh分级的相关性

      【摘要】 目的 比较红细胞分布宽度(RDW)在肝炎后肝硬化、酒精性肝硬化及其他原因肝硬化患者中的变化情况,并探讨RDW与Child-Pugh评分中肝硬化病情严重程度是否相关。方法 选取解放军昆明总医院2015年11月至2017年2月住院患者中诊断为肝硬化的128例为研究对象,其中肝炎后肝硬化68例、酒精性肝硬化21例、其他原因导致的肝硬化 39例。分别收集其RDW及几项主要检验参数值,分析其在不同病因肝硬化患者中的变化情况,以及RDW在不同肝功能Child-Pugh分级的变化特点。结果 肝炎后肝硬化组、酒精性肝硬化组及其他原因肝硬化组患者的RDW分别为(15.09±2.20)%、(15.71±2.53)%、(16.16±2.77)%,三组间比较差异无统计学意义(P>0.05);肝硬化Child-Pugh分级A、B、C三组患者的RDW比较差异亦无统计学意义(P>0.05)。结论 RDW不能鉴别不同病因肝硬化;其与肝硬化肝功能 Child-Pugh分级无相关性,因此 RDW在肝硬化病程中可能不能作为肝功能Child-Pugh分级的参考依据。

Correlations between red blood cell distribution and Child-Pugh classification of liver cirrhosis.

【Abstract】 Objective To compare the changes of red blood cell distribution width (RDW) among posthepatiticliver cirrhosis, alcoholic liver cirrhosis, and liver cirrhosis caused by other reasons, and to determine if RDW follows theseverity of the disease according to Child-Pugh score. Methods A total of 128 hospitalized patients with liver cirrhosis,who admitted to Kunming General Hospital of Chengdu Military Command from November 2015 to February 2017,were selected as the study group. According to etiology, there were 68 cases of posthepatitic liver cirrhosis, 21 cases ofalcoholic liver cirrhosis and 39 cases of cirrhosis due to other causes. The RDW and several main examination parame-ters were collected and analyzed among the three groups with different etiology of cirrhosis, as well as the changed char-acteristics of RDW in different liver function Child-Pugh classification were analyzed. Results The RDW of posthep-atitic liver cirrhosis group, alcoholic liver cirrhosis group and the group caused by other factors were (15.09±2.20)%,(15.71±2.53)%, (16.16±2.77)%, respectively. And no statistically significant difference among them was found (P>0.05). Likewise, there was no significant difference of RDW among hepatic cirrhotic Child-Pugh A, B, and C (P>0.05). Conclusion RDW cannot be used to identify cirrhosis of different etiology. There is no correlation betweenRDW and Child-Pugh grading of liver function in cirrhosis. RDW may not be used as a reference for Child-Pugh classifi-cation in the course of liver cirrhosis.
