      标题:维持性血液透析患者血尿酸水平与心血管疾病的相关性

      【摘要】 目的 探讨维持性血液透析(MHD)患者血尿酸水平与心血管疾病的相关性。方法 选择我院血液净化室在2012年1月至2015年12月期间治疗的72例MHD患者为研究对象,根据血尿酸水平将患者分别纳入正常尿酸组(n=25)和高尿酸组(n=47),比较两组患者的超声心动图心脏结构及功能指标,并比较心血管疾病(CVD)的发病率。结果 本组72例MHD患者中高尿酸血症发生率为65.28%;高尿酸组患者左心室舒张末内径(LVEDD)为(53.13±5.55) mm,明显高于正常尿酸组的(47.38±6.21) mm,舒张早期及晚期最大血流速度比(E/A)、左室射血分数(LVEF)分别为(1.07±0.23)、(51.24±5.32)%,明显低于正常尿酸组的(56.12±6.21)%,差异均有统计学意义(P<0.05);高尿酸组患者CVD发生率为63.83%,明显高于正常尿酸组的40.00%,差异有统计学意义(P<0.05)。结论 血尿酸水平能反映MHD患者心脏结构和功能的变化,对MHD患者左心室肥厚和功能的评估具有重要价值,并且血尿酸水平与心血管疾病的发生关系密切。

Correlation between serum uric acid and cardiovascular disease in maintenance hemodialysis patients.

【Abstract】 Objective To investigate the correlation between serum uric acid level and cardiovascular diseasein maintenance hemodialysis (MHD) patients. Methods From January 2012 to December 2015, 72 MHD patients inBlood Purification Room of our hospital were selected as research objects. According to the level of blood uric acid, theywere divided into normal uric acid group (n=25) and high uric acid group (n=47). Echocardiography in cardiac structureand function, and cardiovascular disease (CVD) rate in two groups were compared. Results In the 72 cases, the hyperuri-cemia rate was 65.28%. Left ventricular end-diastolic diameter (LVEDD) in high uric acid group were (53.13±5.55) mm,significantly higher than (47.38 ± 6.21) mm in the normal uric acid. Left ventricular ejection fractions (LVEF) were(51.24±5.32)%, significantly lower than (56.12±6.21)% in normal uric acid group (P<0.05). CVD rate in high uric acidgroup was 63.83%, significantly higher than 40.00% in normal uric acid group (P<0.05). Conclusion Serum uric acidlevel can reflect the changes of cardiac structure and function in patients with MHD, which is of great value in the evalu-ation of left ventricular hypertrophy and function in patients with MHD, and is closely related to the occurrence of car-diovascular disease.
