      【摘要】 目的 分析体表淋巴结针吸细胞学镜下特点及误诊原因。方法 经术后组织病理学证实，回顾性总
结德阳市人民医院及旌阳区中医院2010年1月至2015年5月445例体表淋巴结针吸细胞学诊断准确率及误诊原
因。结果 445例针吸细胞学诊断正确率为95.51% (425/445)，误诊率为4.49% (20/445)，误诊的主要原因有坏死严
重未见有效成分、过诊断、漏穿、肿瘤细胞少未见典型病变及肿瘤细胞异型性小。结论 体表淋巴结针吸细胞学诊
断准确率较高，不断总结误诊原因可以提高体表淋巴结针吸细胞学诊断准确率。

【Abstract】 Objective To study the diagnostic accuracy of fine needle aspiration cytology (FNAC) for superfi-
cial lymph node enlargement and the reasons of misdiagnosis. Methods The diagnostic accuracy and misdiagnosis rea-
sons of 445 patients with superficial lymph node enlargement, who admitted to the Traditional Chinese Medicine Hospi-
tal of Jingyang District of Deyang from January 2010 to May 2015 were retrospectively analyzed. The patients were con-
firmed by postoperative histopathology. Results The concordance rate between the cytologic and corresponding histo-
logic diagnoses was 95.51% (425/445) and the misdiagnosis rate was 4.49% (20/445). The main causes of misdiagnosis
were serious necrosis, over-diagnosis, leakage, inadequate cellularity and less atypia. Conclusion Fine needle aspira-
tion biopsy of superficial lymph node carries a high diagnostic accuracy, and constant summarization of the misdiagno-
sis causes can improve the diagnostic accuracy of fine needle aspiration cytology of the superficial lymph nodes.
