      标题:经颅多普勒超声联合颈动脉超声诊断缺血性脑血管病的临床价值

      【摘要】 目的 探讨经颅多普勒超声(TCD)与颈动脉超声联合应用对诊断缺血性脑血管病的临床价
。方法 对 109例缺血性脑血管病患者均分别行头颅数字减影血管造影(DSA)检查、经颅多普勒超声和颈动
脉超声检查,观察血管病变情况,以头颅DSA检查结果作为诊断的金标准。结果 经DSA检查,109例患者颅
内、颈部血管均有不同程度的病变,异常血管895条。单用经颅多普勒超声检查发现颅内血管异常86例,异常血
管345条;单用颈动脉超声检查发现颅外血管异常74例,异常血管322条;经颅多普勒超声联合颈动脉超声发现
颅内外异常血管105例,异常血管832条。单用经颅多普勒超声检查、单用颈动脉超声检查、经颅多普勒超声联
合颈动脉超声检查的诊断符合率分别为86.99%%、83.49%和94.32%。结论 经颅多普勒超声联合颈动脉超声能
明显提高颅内外血管病变诊断的准确率,对缺血性脑病的临床诊断有重要的指导意义。

Clinical value of transcranial Doppler ultrasonography combined with carotid ultrasound in the diagnosis of
ischemic cerebrovascular disease.

【Abstract】 Objective To explore the value of transcranial Doppler ultrasonography combined with carotid
ultrasound in the diagnosis of ischemic cerebrovascular disease. Methods One hundred and nine patients of isch-
emic cerebrovascular disease were performed head digital subtraction angiography (DSA), transcranial Doppler ultra-
sonography, carotid ultrasound to observe the vascular lesions. Head DSA was regarded as the gold standard examina-
tion. Results DSA examination showed that all the patients had different degree of pathological changes in head and
carotid, with 895 abnormal blood vessels. Transcranial Doppler ultrasonography found 86 cases of intracranial vascu-
lar abnormalities, with 345 abnormal blood vessels. Carotid ultrasound revealed 74 cases of extracranial vascular ab-
normalities, with 322 abnormal blood vessels. Carotid ultrasound combined with transcranial Doppler ultrasound
found 105 cases of vascular abnormalities, with 832 abnormal blood vessels. The diagnostic coincidence rate of tran-
scranial Doppler ultrasonography, carotid ultrasound, transcranial Doppler ultrasonography combined with carotid ul-
trasound were 86.99%%, 83.49% and 94.32%, respectively. Conclusion Transcranial Doppler ultrasonography com-
bined with carotid ultrasound can significantly increase the correct rate of diagnosing intracranial and extracranial vas-
cular lesions. It plays an important role in the guidance of clinical work.
