      标题:经阴道超高速剪切波对宫颈癌前病变及宫颈浸润癌的诊断价值

      【摘要】 目的 探讨超高速剪切波弹性成像在宫颈癌前病变及宫颈浸润癌诊断中的价值。方法 利用超高速
弹性剪切波对经过筛选的78例已婚成年女性进行诊断,选择其中TCT提示无上皮内瘤样病变者30例,设为A组(正
常组);TCT提示鳞状细胞异常且经病理确诊为癌前病变者(即CINⅠ、Ⅱ、Ⅲ)30例,设为B组;TCT及病理均提示鳞状
细胞癌且临床分期为Ⅰb期者18例,设为C组。所有受试者均进行常规二维彩色多普勒及经阴道超高速剪切波弹性
成像检查,以便获得杨氏模量平均值。结果 A组的超高速剪切波弹性成像杨氏模量平均值为(21.81±3.87) kPa,B
组为(21.27±3.21) kPa,两组比较差异无统计学意义(P>0.05);C组病灶的杨氏模量平均值为(97.55±87.17) kPa,C组
病灶周围正常组织的杨氏模量平均值为(24.47±9.66) kPa,两组比较差异具有统计学意义(P<0.05)。结论 超高速
剪切波弹性成像技术在宫颈癌前病变诊断上没有明显帮助,但能给宫颈浸润癌的诊断提供有价值的信息。

Value of transvaginal shear wave elastography in diagnosis of cervical precancerous lesions and invasive cervical
carcinoma.

【Abstract】 Objective To discuss the value of shear wave elastography (SWE) in the diagnosis of cervical
precancerous lesions and invasive cervical carcinoma. Methods Seventy-eight adult women received conventional
two-dimensional color Doppler and transvaginal SWE to acquire the average value of Young's modulus value. The pa-
tients were divided into three groups: group A (30 patients, no intraepithelial neoplasia disorders by CTC), group B
[(30 patients, with abnormal squamous cell by TCT and were confirmed by pathology as cervical intraepithelial neo-
plasia (CINⅠ, Ⅱ, Ⅲ)], group C (18 patients, squamous-cell carcinoma by both TCT and pathology, with the clinical
stages of Ⅰb). Results The average maximum value of Young's modulus value of SWE was (21.81±3.87) kPa in
group A, (21.27±3.21) kPa in group B, with no statistically significant difference between the two groups (P>0.05). In
Group C, the Young's modulus value of lesion was (97.55±87.17) kPa, and that of normal tissue around the lesion was
(24.47±9.66) kPa, with statistically significant difference (P<0.05). Conclusion Shear wave elastography brings no
significant benefits in the diagnosis of cervical precancerous lesions, but it can provide valuable information for the di-
agnosis of invasive cervical carcinoma.
