      标题:B超引导下小针刀治疗腰脊神经后支综合征的疗效及其对患者术后疼痛的影响

      【摘要】 目的 探讨B超引导下小针刀治疗腰脊神经后支综合征的疗效及其对患者术后疼痛的影响。
方法 选取 2013年 1月至2014年6月期间我院确诊治疗的腰脊神经后支综合征患者118例,依据随机分配原则
分为B超引导组和X线引导组,每组59例,X线引导组患者给予在X线片引导下小针刀治疗,B超引导组患者给
予在B超引导下小针刀治疗,对所有患者治疗前、治疗后 4周内应用视觉模拟评分法(Visual Analogue Scale/
Score,VAS)进行疼痛评估和进行为期90 d的随访,统计分析所有患者治疗中穿刺成功情况、治疗前后VAS评分
和疗效。结果 B超引导组患者总穿刺成功率明显高于X线引导组,差异有统计学意义(P<0.05);治疗90 d后,B
超引导组患者治疗总有效率明显高于X线引导组,差异有统计学意义(P<0.05);治疗前,两组患者VAS评分比较
差异无统计学意义(P>0.05);治疗后,B超引导组患者各时点VAS评分明显低于X线引导组,差异有统计学意义
(P<0.05)。结论 B超引导下小针刀治疗可有效提高腰脊神经后支综合征治疗中穿刺成功率,缓解治疗后患者
疼痛症状,有助于提高对患者的疗效,值得临床推广应用。

Effect of B-ultrasound guided small needle knife in the treatment of syndrome in the posterior ramus of
lumbar spinal nerve and its effect on postoperative pain of patients.

【Abstract】 Objective To discuss the effect of B-ultrasound guided small needle knife in the treatment of
syndrome in the posterior ramus of lumbar spinal nerve, and to analyze its effect on postoperative pain of patients.
Methods A total of 118 patients with syndrome in the posterior ramus of lumbar spinal nerve in our hospital from Jan.
2013 to Jun. 2014 were selected, which were randomly divided into B-ultrasound guided group and X-ray guided group,
with 59 cases in each group. Patients in X-ray guided group were treated with small needle knife therapy under X-ray
guidance, while those in B-ultrasound guided group were treated with small needle knife under B-ultrasound guidance.
Pain assessment was performed in all the patients before treatment and 4 weeks after treatment based on Visual Analogue
Scale (VAS). The patients were followed up for a period of 90 d. The puncture success situation, VAS scores before and
after treatment and curative effect were statistically analyzed. Results The total puncture success rate was significantly
higher in B-ultrasound guided group than X-ray guided group (P<0.05). 90 d after treatment, the total effective rate was
significantly higher in B-ultrasound guided group than X-ray guided group (P<0.05). Before treatment, VAS score
showed no significant difference between two groups (P>0.05), but the score after treatment in B-ultrasound guided
group was significantly lower (P<0.05). Conclusion B-ultrasound guided small needle knife therapy can effectively
improve the success rate of puncture in syndrome in the posterior ramus of lumbar spinal nerve, relieve patients' pain
symptoms after treatment, and help to improve the curative effect, which is worthy of further clinical promotion.
