      标题:斑点追踪二尖瓣环位移成像技术评估精神分裂症患者左心收缩功能

      【摘要】 目的 应用二维斑点追踪成像(Two-dimensional speckle tracking imaging,2D-STI)组织二尖瓣瓣环
位移 (Tissue mitral annular displacement, TMAD)技术评估精神分裂症患者左心收缩功能。方法 研究对象为精
神分裂症患者83例(病例组),70例健康志愿者(对照组)。应用常规超声心动图检查,简易双平面Simpson法测量
左室射血分数(Left ventricular ejection fraction,LVEF),应用Qlab9.0工作站,自动获取二尖瓣瓣环4个位点收缩期
向心尖方向的最大位移TMAD、二尖瓣瓣环连线中点最大位移(TMADMidpt)及其占左室舒张末期长径的百分
比 (TMAD Midpt%)的均值进行 t检验分析,并与LVEF进行Pearson相关分析。结果 病例组与对照组LVEF差
异无统计学意义(P>0.05)。与对照组比较,病例组的TMAD、TMADMidpt、TMAD Midpt%均降低,差异有统计学
意义(P<0.05)。两组LVEF与TMAD、TMADMidpt、TMAD Midpt%有相关性(对照组:r=0.643、0.663、0.707;病例
组:r=0.620、0.634、0.663;P均<0.001)。结论 TMAD指标可较敏感反映精神分裂症患者早期左室收缩功能变
化,STI技术测定二尖瓣瓣环位移在精神分裂症患者的心功能评估中具有一定的价值。

Assessment of left ventricular systolic function in patients with schizophrenia by tissue mitral annular
displacement.

【Abstract】 Objective To assess the left ventricular systolic function in patients with schizophrenia by tissue mi-
tral annular displacement (TMAD) based on two-dimensional speckle tracking imaging (2D-STI). Methods Eighty-three
patients with schizophrenia (case group) and seventy healthy volunteers (control group) were enrolled in this study. All the
participants received routine echocardiography examinations. Left ventricular ejection fraction (LVEF) was calculated
by biplane Simpson method. Mitral displacement was determined using the TMAD feature of Philips QLAB version 9.
TMAD parameters included the maximal displacement of the four points of mitral annulus (TMAD), the middle point
of mitral annulus (TMADMidpt) and its ratio to the length of left ventricular at end-diastole (TMAD Midpt%), All da-
ta were reported as the average of the measurements of four sites of the mitral annulus. Differences between groups
were analyzed using the Student's t test .The relationship of these displacements parameter with LVEF was expressed
by Pearson correlation coefficients. Results The LVEF had no significant difference between the two groups
(P>0.05). Compared with the control group, the TMAD, TMADMidpt and TMAD Midp% were significantly de-
creased in patients with schizophrenia (P<0.05). All these three parameters correlated well with EF in each group (con-
trol group: r=0.643, 0.663, 0.707; case group: r=0.620, 0.634, 0.663 respectively, P<0.001). Conclusion TMAD
could indicate the early change of left ventricular systolic function in patients with schizophrenia sensitively. It has a
certain value in assessment of the left ventricular systolic function in patients with schizophrenia.
