      【摘要】 目的 检测CD56、CD54在甲状腺病变组织中的表达情况,探讨CD56、CD54的表达在甲状腺病变鉴别
诊断中的作用。方法 分别收集海南医学院附属医院病理科 73例甲状腺乳头状癌、64例良性病变组织蜡块。
将 137例石蜡标本制做组织芯片。应用免疫组织化学技术检测CD56、CD54蛋白在甲状腺病变组织中的表达情
。结果 CD56在甲状腺乳头状癌组织中大多表达缺失,表达率为 14%,在良性甲状腺病变组织中表达率高
(92.2%),两者间表达率差异具有显著统计学意义(P<0.01)。CD54在甲状腺乳头状癌及桥本氏甲状腺炎组织中表
达增高(69.9% vs 75.0%),呈弥漫阳性表达,这两组间表达差异无统计学意义(P>0.05);在其他良性甲状腺组织包
括结节性甲状腺肿和滤泡性腺瘤中,CD54表达较低(3.8%),主要呈散在弱阳性表达,与甲状腺乳头状癌组织相比,
其表达差异具有显著统计学意义(P<0.01)。结论 CD56在甲状腺乳头状癌中表达缺失,在甲状腺良性病变组织
中表达增高;CD54在甲状腺乳头状癌及桥本氏甲状腺炎中高表达,联合检测CD56、CD54对甲状腺病变的鉴别诊断
具有重要作用。

【Abstract】 Objective To detect the expression levels of CD56 and CD54 in different thyroid lesions, and dis-
cuss the application of CD56 and CD54 in the differential diagnosis of thyroid lesions. Methods Paraffin samples in-
cluding 73 cases of papillary thyroid carcinoma (PTC) and 64 cases of thyroid benign hyperplasia tissues from pa-
thology department of Hainan medical college affiliated hospital were collected to be made as tissue chips. And then
immunohistochemical techniques were applied to detect the expression levels of CD56 and CD54 in thyroid lesions.
Results The expression level of CD56 in benign tissue was higher than that in PTC (92.2% vs 14%, P<0.01). Com-
pared with the groups of nodular goiter and follicular adenoma, the CD56 positive expression rate in PTC was higher
with a significant difference (69.9% vs 3.8%, P<0.01). However, there was no difference in the positive expression
rate of CD56 between PTC and Hashimoto's thyroiditis (69.9% and 75.0%, P>0.05). Conclusion With lower expres-
sion of CD56 in PTC rather than in benign tumor and higher expression of CD54 in PTC and Hashimoto's thyroiditis, the
combined detection of CD56 and CD54 might contribute to differential diagnosis of the thyroid diseases.
