      标题:面神经管64排螺旋CT低剂量扫描的临床应用

      【摘要】 目的 探讨面神经管64排螺旋CT低剂量扫描临床应用的可行性。方法 收集2015年3~9月临床
怀疑颞骨、鼻副窦病变行64排螺旋CT且无面神经疾病患者88例,采用随机单盲法分成常规剂量组(A组)和三组不
同扫描参数的低剂量组(B-D组),每组22例。常规剂量组使用参数为120 kV/240 mA,低剂量组的扫描参数分别为
B组(100 kV/240 mA)、C组(100 kV/120 mA)、D组(100 kV/80 mA)。采集图像由2名放射诊断主治医师及以上人员
共同进行分析,包括图像质量评分、记录图像噪声和辐射剂量。图像质量评分的统计分析使用 Kruskal-Wallis H秩
和检验,噪声分析使用单因素方差分析。结果 A、B、C、D四组的图像质量评分分别为20.0分(19.5,20.0)、19.5分
(19.5,20.0)、19.5分 (19.5,20.0)和 19.5分 (19.5,20.0),CT值分别为 (2 258.43±90.65) Hu、(2 289.00±43.89) Hu、
(2 298.82±46.86) Hu、(2 299.89±48.05) Hu,四组比较差异均无统计学意义(P>0.05),图像能够满足诊断要求;B、C、D
三组CT剂量指数、有效剂量E与常规剂量组比较均有不同程度的降低,以D组分别下降79.3%、79.2%最为显著,患
者接受的辐射剂量明显降低。结论 100 kV/80 mA可作为64排螺旋CT面神经管低剂量扫描优化参数阈值。

Clinical application of low-dose scan solution in 64-slice spiral computed tomography of the facial canal.

【Abstract】 Objective To explore the clinical feasibility of low dose scan solution in 64-slice spiral computed
tomography of the facial canal. Methods From March 2015 to September 2015, a total of 88 patients suspected of
temporal bone and paranasal sinus lesions, who had no facial nerve disease and underwent 64-slice spiral CT scan, were
randomly divided into routine dose group (A group, 120 kV/240 mA) and 3 different scan parameters of low-dose groups
[B group (100 kV/240 mA), C group (100 kV/120 mA), D group (100 kV/80 mA)], with 22 cases in each group. The col-
lected images were evaluated by at least 2 diagnostic radiology doctors in terms of image quality score, recording im-
age noise and radiation dose. Image quality score and image noise were analyzed respectively by Kruskal-Wallis H
test and one-way ANVOA. Results The image quality scores of A, B, C, D groups were respectively 20.0 points
(19.5, 20.0), 19.5 points (19.5, 20.0), 19.5 points (19.5, 20.0), 19.5 points (19.5, 20.0); The CT values were respectively
(2 258.43±90.65) Hu, (2 289.00±43.89) Hu, (2 298.82±46.86) Hu, (2 299.89±48.05) Hu. There were no statistically signifi-
cant differences in our groups in term of image quality scores and CT values (P>0.05). The images of the four groups all can
satisfy the diagnostic requirements. Compared with routine dose group, the CT dose index and the effect dose E in B, C, D
groups had different degrees of reduction, among which D group had the most significant decreases by 79.3% and
79.2%, respectively. The received radiation dose of patients was significantly reduced. Conclusion The scanning condition
of 100 kV/80 mA can be used as a facial canal low-dose scan threshold optimization parameter in 64-slice spiral CT.
