      标题:两种手术方法治疗跟骨骨折对比研究

      【摘要】 目的 比较经皮撬拨复位石膏固定和切开复位钢板内固定治疗跟骨骨折的效果。方法 对2009
年3月至2012年3月收治的50例跟骨骨折患者进行回顾性分析,其中26例采用经皮撬拨复位石膏固定(A组),24
例采用切开复位钢板内固定(B组),比较治疗效果并对其进行评估。结果 所有随访患者均骨性愈合,并获得
10~24个月随访,平均随访14.6个月。通过测定治疗前后跟骨B?hler角及Gissane进行比较,按Maryland评分,A
组优良率为80.8%,B组为87.5%,两组优良率比较差异无统计学意义(P>0.05)。结论 两种方法可获得相似的治
疗效果,经皮撬拨复位石膏固定特别适应于软组织条件及经济条件较差的患者,可以最大限度降低术后的切口
皮缘坏死及感染风险,降低患者经济负担。若软组织条件较好,可选用切开复位钢板内固定术,利用早期进行功
能锻炼。

Comparison of two methods for the treatment of calcaneal fractures.

【Abstract】 Objective To compare the efficacy of percutaneous poking reduction with plaster fixation and
open reduction plate internal fixation for the treatment of calcaneal fractures. Methods From March 2009 to March
2012, 50 cases with calcaneal fractures were enrolled into this study, of which 26 cases treated with percutaneous poking
reduction and plaster fixation (group A),while 24 were treated with open reduction and internal fixation (group B). The
curative effects were assessed and analyzed retrospectively. Results All patients had been followed up for 10~24
months (average of 14.6 months). The calcaneal B?hler and Gissane angle was determined before and after treatment,
according to the Maryland, and excellent and good rates of group A and group B were 80.8% and 87.5% respectively.
Conclusion Two methods have similar therapeutic efficacy. Percutaneous poking reduction plaster fixation can mini-
mize postoperative skin flap necrosis and infection risk, and reduce the economic burden of the patients, and it adapt
to poor the soft tissue conditions and poor economic conditions in patients. If the soft tissue conditions are better, open
reduction and plate fixation can be chosen, which may be useful for early functional exercise.
