      标题:急危重症患者动脉血与静脉血电解质钾相关性的研究

      【摘要】 目的 探讨急危重患者动脉血钾与静脉血钾浓度的相关性,为临床救治提供参考。方法 采用前
瞻性研究方法,选择我院2012年5~12月间重症监护室收治的急危重合并血钾异常患者66例,根据血钾值分为两
组,低血钾组 33例和高血钾组 33例,分别经动脉留置针采集抗凝动脉血和常规穿刺静脉采集静脉血测定血钾
浓度,比较两种方法测定结果及生物参考区间。结果 急危重症患者动脉血钾平均浓度明显低于静脉血钾
浓度,两种测定方法比较差异具有统计学意义(P<0.05),其中低血钾组生物参考区间为(0.21±0.03) mmol/L。
高血钾组生物参考区间为(0.35±0.04) mmol/L。结论 经动脉留置针采集动脉血作床旁血气分析监测急危重
症患者血钾具有方便、快捷、可靠的优势,掌握动脉血钾与静脉血钾的生物参考区,对及时诊断急危重症患者
钾异常具有积极的意义。

【Abstract】 Objective To study the correlation of electrolyte of potassium in arterial blood and venous blood
of critical patients, providing reference for clinical treatment. Methods According to prospective study method, 66
acute critical patients with potassium abnormality from the First People's Hospital of Zigong City from May 2012 to
December 2012 were enrolled in the study. According to the serum potassium values, all patients were divided into
two groups: 33 cases in hypokalemia group and 33 cases in hyperkalemia group. The two groups were collected antico-
agulant arterial blood and venous blood by arterial indwelling needle to determinate potassium concentration. The de-
termination results and biological reference range of two methods were compared. Results The average concentra-
tion of potassium in arterial blood was lower than that in venous blood, with statistically significant difference (P<
0.05). The biological reference range of hypokalemia group was (0.21±0.03) mmol/L, and that of hyperkalemia group
was (0.35±0.04) mmol/L. Conclusion Collecting arterial blood by arterial indwelling needle as a bedside blood gas
analysis to monitor potassium concentration of acute critical patients is convenient, speedy and reliable. Mastering the
biological reference range of artery blood potassium and venous blood potassium has positive significance for timely
diagnosis of potassium abnormality in acute critical patients.
