      标题:不同剂量地佐辛对瑞芬太尼静脉全麻术后苏醒期的影响

      【摘要】 目的 研究不同剂量地佐辛对腹腔镜手术患者瑞芬太尼静脉全麻术后苏醒期的影响。方法
择 100例腹腔镜手术患者,按随机数字表法分为实验组四组(T1组、T2组、T3组、T4组)和对照组(C组),每组 20
例,行双盲试验。在麻醉深度监测下行麻醉诱导及维持,维持药物为丙泊酚+瑞芬太尼,术毕前15 min,T1组、T2
组、T3组、T4组分别静注地佐辛80 μg/kg、60 μg/kg、40 μg/kg、20 μg/kg,C组静注生理盐水10 ml,观察患者睁眼时
间、呼吸恢复时间、拔管时间、疼痛视觉模拟评分(VAS)、恶心呕吐及躁动不良反应。结果 T1组、T2组的睁眼时
间、呼吸恢复时间、拔管时间明显较T3组、T4组和C组延长(P<0.05),T4组和C组VAS评分及躁动情况发生率显
著高于T1组、T2组和T3组(P<0.05)。结论 瑞芬太尼静脉全麻腹腔镜手术患者,地佐辛(40 μg/kg)可预防术毕
疼痛及痛觉过敏现象,减少苏醒期的躁动。

Effects of dezocine of different doses on remifentanil intravenous general anesthesia postoperative revival
period under the depth of anesthesia monitoring.

【Abstract】 Objective To research the effects of dezocine of different doses on remifentanil intravenous gen-
eral anesthesia postoperative revival period under the depth of anesthesia monitoring. Methods A total of 100 lapa-
roscopic surgery patients were selected and divided into 4 experimental groups (T1,T2,T3,T4) and control group
(group C) randomly, with 20 cases in each group. Administer induction and maintenance of anesthesia were performed
by using remifentanil and propofol under the depth of anesthesia monitoring. Ten minutes before the end of surgery, in-
travenous dezocine to the group T in the units of 80 μg/kg, 60 μg/kg, 40 μg/kg, 20 ug/kg respectively, 10 ml of saline to
group C. Recovery time of spontaneous breathing, eye opening time, extubation time, alert/sedation score (OAA/S), vi-
sual analogue scale (VAS) and adverse reactions of nausea, vomiting and dysphasia were observed. Results The re-
covery time of spontaneous breathing, eye opening time and extubation time in group T1 and T2 were longer than those
in group T3, T4 and C significantly (P<0.05), but OAA/S was lower (P<0.05). The VAS and dysphoria incidence in
group T4 and C were significantly higher than those in other groups. Conclusion Appropriate dose of dezocine can ef-
fectively prevent pain and hyperalgesia due to remifentanil' short half-life, reduce dysphoria in the awakening period,
decrease adverse reactions and improve patient satisfaction.
