      标题:AFP-L3、AFU和HCY联合检测对原发性肝癌的临床诊断价值

      【摘要】 目的 探讨血清肿瘤标志物甲胎蛋白异质体(AFP-L3)、a-L岩藻糖苷酶(AFU)和同型半胱氨酸(HCY)
联合对原发性肝癌(PHC)的诊断价值。方法 PHC组 215例,良性肝病组178例,正常对照组80例。首先采用微量
离心柱法分离得到AFP-L3,后采用化学发光法检测AFP-L3含量及血清AFP总量,再计算AFP-L3的比值,即
AFP-L3占血清AFP总量的百分比。采用CNPF底物法检测血清AFU,采用循环酶法检测血清HCY。结果 PHC
患者血清中AFP-L3、AFU和HCY水平均明显高于良性肝病组和正常对照组(P均<0.05)。血清中三种肿瘤标志
物单项检测的敏感性分别为 84.2%、78.1%、56.7%,联合检测后敏感性提高到了 88.4%,与单项检测的敏感性比
较,χ2值分别为4.080、8.074、54.007,P均<0.05,差异均具有统计学意义。结论 血清AFP-L3、AFU和HCY是临
床诊断PHC较好的肿瘤标志物,三项联合检测有助于PHC的早期诊断和鉴别诊断。

【Abstract】 Objective To evaluate the clinical diagnostic value of combined detection of serum tumor mark-
ers, Alpha-fetoprotein heterogeneity (AFP-L3), α-L-fucose-glucoside (AFU), homocysteine (HCY) for primary hepat-
ic carcinoma (PHC). Methods A total of 215 patients of PHC (PHC group), 178 patients of benign liver disease (be-
nign liver disease group), 80 healthy people (the control group) were enrolled in the study. Microspincolumn was ap-
plied to isolate AFP-L3, then the AFP-L3 content and total serum AFP were detected by chemiluminescence. The per-
centage of AFP-L3 in total serum AFP was calculated. CNPF assay was applied to detect serum AFU, and cyclic enzy-
matic detection was used to detect serum HCY. Results The serum levels of AFP-L3, AFU, HCY in PHC group
were significantly higher than those of benign liver disease group and the control group (P<0.05). The sensibility of
three tumor markers, AFP-L3, AFU, HCY for PHC was 84.2%, 78.1%, 56.7% in single detection, and was 88.4% in
combined detection. Moreover, in comparison with the single detection, the values of χ2 were 4.080, 8.074, 54.007, re-
spectively (P<0.05). The differences were statistically significant. Conclusion Serum AFP-L3, AFU and HCY of
PHC are better tumor markers in clinical diagnosis, and combined detection of AFP-L3, AFU, HCY is helpful to the
early diagnosis and differential diagnosis of PHC.
