      标题:妊高征患者胰岛素抗性的临床变化及意义

      【摘要】 目的 研究妊高征(PIH)患者胰岛素抗性的临床变化及其与血糖、血脂指标变化的关系,分析胰岛
素抗性与妊高征的相关性。方法 选取我院30例妊高征产妇(实验组)和30例正常妊娠产妇(对照组)作为研究对
象,比较两组产妇各时间血糖与胰岛素水平变化及空腹血脂各项指标[(包括总胆固醇(TC)、载脂蛋白A-1
(ApoA-1)、载脂蛋白B(ApoB)、甘油三酯(TG)、高密度脂蛋白(HDL)和低密度脂蛋白(LDL)]。结果 实验组30例
产妇口服葡萄糖3 h血糖值(SG)均高于对照组,差异具有统计学意义(P<0.05),其余时间差异无统计学意义;除
空腹外,实验组各时间胰岛素水平均高于对照组,差异具有统计学意义(P<0.05或P<0.01);实验组胰岛素与血
糖值比值均高于对照组,除第30分钟外,其他时间差异均有统计学意义(P<0.05或P<0.01);HOMA指数与其他
血脂指标均有相关性,实验组产妇HOMA、TG、LDL、ApoB水平均明显高于对照组,差异具有统计学意义(P<0.
05);而HDL和ApoA-1水平明显低于对照组,差异具有统计学意义(P<0. 05)。结论 胰岛素抵抗和血脂异常是
妊高征发生的重要因素,且二者间有相关性。

Clinical changes and significance of insulin resistance in patients with pregnancy induced hypertension.

【Abstract】 Objective To investigate the changes of insulin resistance in patients with pregnancy induced hy-
pertension (PIH), to study its relationship with blood glucose, blood lipid, and to analyze the correlation between insu-
lin resistance and PIH. Methods Thirty PIH patients (experimental group) and 30 normal pregnant women (control
group) were selected as research subjects. The blood glucose, insulin levels, the fasting lipid indexes (TC, ApoA-1,
ApoB, TG, HDL and LDL) were compared between the two groups. Results Three hours after oral intake of glu-
cose, the SG in the experimental group was significantly higher than the control group (P<0.05), with no statistically
significant difference at other time points. The insulin levels in the experiment group were significantly higher than
that in the control group at all time points except fasting (P<0.05 or P<0.01). The ratio of insulin to blood sugar levels
in the experiment group were significantly higher than that in the control group, except the 30 min (P<0.05 or P<
0.01). HOMA, TG, LDL, ApoB levels in the experiment group were significantly higher than those in the control
group (P<0.05), but HDL ApoA-1 was significantly lower (P<0.05). Conclusion Insulin resistance and dyslipid-
emia are important factors for pregnancy induced hypertension.
