      标题:原发性高血压肾病患者血清胱抑素C和视黄醇结合蛋白检测的临床意义

      【摘要】 目的 探讨原发性高血压肾病患者血清胱抑素C (Cysc)和血清视黄醇结合蛋白(RBP)水平检测
的临床意义。方法 应用免疫比浊法对184例原发性高血压患者(正常蛋白尿组57例,微量蛋白尿组59例,大
量蛋白尿组 68例)血清Cysc、RBP以及尿液微量蛋白(MALB、IgG、TRFU、α1-MG、β2-MG)的水平进行检测,并
与 72例健康体检者(正常对照组)进行对比分析。 结果 微量蛋白尿组和大量蛋白尿组患者血清Cysc、RBP
尿MALB、IgG、TRFU、α1-MG、β2-MG等各项微量蛋白水平显著高于正常对照组和正常蛋白尿组,差异有统计
学意义(P<0.05)。原发性高血压肾病患者血清Cysc与血清RBP及各项尿MALB、IgG、TRFU、α1-MG、β2-MG等微
量蛋白指标水平呈正相关(r=0.92、0.85、0.76、0.67、0.58、0.63,P<0.05)。结论 血清Cysc与RBP及各项尿ALB、
IgG、TRFU、α1-MG、β2-MG等微量蛋白含量随原发性高血压肾病的发生以及严重程度逐渐增高,可作为早期诊断
和监测原发性高血压肾病敏感的指标。

Clinical significance of serum cystatin C and retinol binding proteinin in patients with primary hypertension
nephropathy.

【Abstract】 Objective To explore the clinical significance of serum cystatin C (CYSC) and retinol binding
proteinin (RBP) in patients with primary hypertension nephropathy. Methods Serum CYSC, RBP and urine micro-
protein levels (MALB, IgG, TRFU, α 1-MG, β 2-MG) were detected with immunoturbidimetry in 184 patieents (57
cases in the normal albuminuria group, 59 cases in the microalbuminuria group and 68 cases in the macroalbumin-
uria group) with primary hypertension nephropathy, and compared with those of 72 healthy voluntes (the control
group). Results The levels of CYSC, RBP, MALB, IgG, TRFU, α1-MG and β2-MG in the microalbuminuria group and
68 macroalbuminuria group were significantly higher than those in the control group and the normal albuminuria group (P<
0.05). Serum CYSC had positive correlation with RBP, MALB, IgG, TRFU,α1-MG and β2-MG (r=0.92, 0.85, 0.76, 0.67,
0.58, 0.63, P<0.05) in the patients. Conclusion The levels of CYSC, RBP, M ALB, IgG, TRFU, α1-MG and β2-MG in-
crease gradually with the primary hypertension nephropathy getting more severe. It could be used as a sensitive mark-
er for early monitoring the development and progress of primary hypertension nephropathy.
