      标题:七氟烷和丙泊酚麻醉对老年结直肠癌患者认知功能的影响

      【摘要】 目的 比较七氟烷和丙泊酚麻醉对老年结直肠癌患者认知功能的影响。方法 选择2010年7月
至 2012年 7月在我院普外科住院治疗的 126例老年结直肠癌患者为研究对象,运用随机数字表法将符合纳入
标准和排除标准的患者分为A组和B组,A组患者麻醉诱导后静脉靶控输注丙泊酚维持麻醉,B组患者麻醉诱
导后给予吸入七氟烷维持麻醉,采用简易精神状态量表(MMSE)评价患者的认知功能变化和其他药物不良反应
情况。结果 B组患者术后应答后 1 h、6 h和 12 h的评分均明显低于A组患者相应时点的评分(P<0.05),但是,
在应答后 24 h,两组患者的MMSE评分恢复至麻醉前水平。B组患者恶心呕吐和眩晕发生率明显低于A组患
者 (P<0.05),对手术满意度明显高于A组患者(P<0.05)。结论 与丙泊酚麻醉相比,吸入七氟烷维持麻醉老年结
直肠癌术后,患者会出现可逆性的认知功能障碍,但是麻醉安全,不良反应少。

Effect of sevoflurane and propofol anesthesia on the cognitive function of elderly patients with colorectal
cancer.

【Abstract】 Objective To investigate the effect of sevoflurane and propofol anesthesia on the cognitive func-
tion of elderly patients with colorectal cancer. Methods The clinical data of 126 elderly patients with colorectal can-
cer who were treated in the Department of General Surgery in our hospital from July 2010 to July 2012 were collected,
The patients were divided into group A and group B according to random number table. Patients in group A were main-
tained by propofol after induction of anesthesia, but patients in B group was maintained by sevoflurane after induction
of anesthesia, The patients were surveyed by self-designed general data questionnaire and Mini Mental State Examina-
tion (MMSE) for cognitive function. The adverse reactions were observed in the two groups. Results The scores of
MMSE in group B were lower than that in group A one hour, six hours and twelve hours after response (P<0.05), but
the scores of the two groups recovered to the level before anesthesia twenty-four hours after response. The incidences
of nausea, vomiting and dizziness in group B was significantly lower than those in the group A (P<0.05). The pa-
tients'satisfaction degree in group B was significantly higher than that in group A (P<0.05). Conclusion Sevoflurane
inhalation anesthesia can cause reversible cognitive dysfunction in elderly patients with colorectal cancer, which is
safe and with less adverse reactions.
