      标题:慢性阻塞性肺病急性加重期CRP、CK-MB、LDH水平变化的临床意义

      【摘要】 目的 探讨血清C-反应蛋白(CRP)、磷酸肌酸激酶同工酶(CK-MB)和乳酸脱氢酶(LDH)在慢性
阻塞性肺病(COPD)急性加重期浓度变化及相关临床意义。方法 测定60例COPD患者急性加重期血清CRP、
CK-MB和LDH水平和动脉血氧分压(PaO2),45例健康体检者血清CRP、CK-MB和LDH水平。结果 急性期
COPD患者血清CRP、CK-MB和LDH水平与健康体检者比较差异均有统计学意义(P<0.05)。相关性分析结果
显示,COPD急性期患者CRP与CK-MB、LDH之间及CK-MB与LDH之间呈正相关(P<0.05),COPD急性期患者
PaO2与CRP、CK-MB、LDH之间呈负相关(P<0.01)。结论 COPD急性加重期血清CRP浓度高,机体存在血氧饱
和度低、心肌功能异常及严重感染;联合检测CRP、CK-MB和LDH浓度可以为COPD的病情评估、治疗及预后提
供相关的评价指标。

【Abstract】 Objective To investigate the changes in the serum levels of C-reactive protein (CRP), creatine ki-
nase MB isoenzyme (CK-MB) and lactate dehydrogenase (LDH) in patients with chronic obstructive pulmonary dis-
ease (COPD) at acute exacerbation, and to analyze their clinical significance. Methods The serum levels of CRP,
CK-MB, LDH and PaO2 were measured in 60 patients with COPD at acute exacerbation (the study group) and 45
healthy subjects (the control group). Results The serum levels of CRP, CK-MB, and LDH in the study group were
significantly higher than those in the control group (P<0.05). In the study group, positive correlation was found be-
tween CRP and CK-MB, LDH, as well as between CK-MB and LDH (P<0.01), while negative correlation was found
between PaO2 and CRP, CK-MB, LDH (P<0.01). Conclusion Combined detection of serum CRP, CK-MB and LDH
can provide reliable indicators for the diagnosis, prevention and treatment of COPD.
