      【摘要】 目的 探讨窦镰旁脑膜瘤的显微手术治疗方法。方法 回顾分析我院38例行显微手术治疗的窦
镰旁脑膜瘤患者的临床资料。结果 Ⅰ级切除10例(26.3%)，Ⅱ级切除21例(55.3%)，Ⅲ级切除7例(18.4%)。10
例上下肢运动或感觉障碍者，7例恢复正常，2例未能恢复，1例运动功能障碍加重。8例癫痫者经服药后仅 2例
偶有发作。2例精神障碍者均恢复正常。全部病例无手术死亡。病理结果：内皮型 12例，成纤维型 5例，血管
型 7例，砂粒型 6例，混合型 7例，间变型 1例。术后随访 3个月~5年，4例失访，4例复发，其中 2例再次手术，2
例拒绝手术，行伽玛刀治疗。结论 显微手术是治疗窦镰旁脑膜瘤一种理想的手术方法，值得在临床中广泛
使用并推广。

br>Experience on 38 cases of parafalx meningiomas treated by microsurgery.

【Abstract】 Objective To explore the experience of microsurgery in the treatment of parafalx meningiomas.
Methods The clinical data of 38 patients with parafalx meningiomas in our hospital treated with microsurgery were
retrospectively analyzed. Results ClassⅠ resection were performed in 10 cases (26.3%), classⅡ resection were in
2 cases (55.2%), with class Ⅲ in 7 (18.4%). Of the 10 patients with motor dysfunction and sensory disturbance, 7
were successfully treated and returned to normal, 2 failed to restore normal motor and sensory functions, and 1 was
found with deterioration in motor dysfunction. Of the 8 patients of seizure, only two suffered occasional attacks after
treatment. The two patients with mental disorder were cured. No operative death occurred. The pathologic results re-
vealed endothelial type in 12 cases, fibroblastic type in 15 cases, vascular type in 7 cases, pasmmomatous type in 6
cases, mixed type in 7 cases, and angioblastic type in one case. During the follow-up of 3 months to 5 years, 4 cases
lost follow-up, 4 cases were found with recurrence, two cases received reoperation, and two cases refusing surgery
were treated with gamma knife surgery. Conclusion Microsurgery is an ideal treatment for parafalx meningiomas,
which is worthy to be widely applied in clinical practice.
