      【摘要】 目的 观察0.375%罗派卡因复合小剂量(1.5 mg)吗啡用于硬膜外阻滞行乳腺癌改良根治术的临
床效果。方法 选择ASAⅠ~Ⅱ级择期硬膜外阻滞行乳腺癌改良根治术患者75例，随机分为A（0.375%布吡卡
因）、B（0.375%罗派卡因）、C（0.375%罗派卡因复合小剂量吗啡）三组，各25例。观察记录三组患者术中、术后镇
痛情况和不良反应。结果 三组患者术中镇痛均满意，不良反应发生率B、C组比A组低，术后伤口疼痛C组比
A、B组轻。结论 0.375%罗哌卡因复合小剂量吗啡用于乳腺癌改良根治术，安全有效，经济实惠，值得推广。

【Abstract】 Objective To investigate the effects of 0.375% ropivacaine combined with low dose (1.5 mg)
morphine on epidural anesthesia in modified radical operation for breast cancer patients. Methods Seventy-five pa-
tients of breast cancer of ASAⅠ~Ⅱ level, scheduled for epidural anesthesia for modified radical operation, were ran-
domly divided into three groups: Group A (treated by 0.375% bupivacaine), Group B (treated by 0.375 % ropiva-
caine), Group C (treated by 0.375% ropivacaine combined with low dose morphine), each with 25 cases. The analge-
sic efficacy and adverse reactions of patients in the three groups during and after surgery were observed. Results
The analgesic effect showed no statistically significant difference during the operation in the three groups. The inci-
dence of adverse reactions in Group B and Group C were significantly lower than that in Group A, while the postopera-
tive wound pain in Group C was significantly lighter than that in Group A and Group B. Conclusion It is safe, effec-
tive and economical for breast cancer patients to use 0.375% ropivacaine combined with low dose morphine in modi-
fied radical operation, which is worthy to be popularized.
