标题：超声引导经皮肝脏穿刺活检价值研究

      【摘要】目的探讨超声引导下经皮肝脏穿刺活检的临床价值。方法在超声引导下，用Bard 活检枪及
18 G活检针经皮对142 例患者肝脏进行穿刺取肝组织进行病理学检查。根据组织病理学检查结果计算影像学诊
断的符合率，总结操作经验及分析不良反应。结果142 例患者均穿刺成功，获取肝组织302 块。无出血、胆汁
漏及气胸等严重并发症发生。36 例肝细胞癌和9 例肝内胆管癌手术后病理与活检病理结果一致。活检前影像
学诊断总符合率为61.3% (87/142)，与组织病理学检查结果（100%）比较差异有统计学意义(P < 0.001)。结论超
声引导下肝脏穿刺活检安全有效、适应证广、结果可靠，具有重要临床应用价值。

【Abstract】Objective To evaluate the clinical significance of ultrasound-guided percutaneous liver biopsy.
Methods Guided by real-time ultrasound, 142 patients with inclusive liver diseases were undertaken percutaneous
liver biopsy using 18 G core needle. The diagnosis of medical imaging and the results of histopathology were compared,
and the conformity rate was calculated, with the experience and adverse reactions analyzed. Results Biopsy
was performed successfully in all the patients and 302 samples of the liver were obtained. No serious complications occurred,
such as bleeding, bile leakage and pneumothorax. The diagnosis by medical imaging in 36 patients with hepatocellular
carcinoma and 9 patients with intrahepatic cholangiocarcinoma were in accordance with the results of histopathology.
The conformity rate of the medical imaging diagnosis was 61.3% (87/142), significantly lower than that of
histopathology (100%) (P<0.001). Conclusion Ultrasound-guided percutaneous liver biopsy is safe, effective and reliable,
which is of great clinical value.
