标题：超声刀在甲状腺开放性手术中的应用体会

      【摘要】 目的 探讨超声刀在开放性甲状腺手术中的应用价值。方法 收集我院 2008年 8月至 2010年 5
月同一手术小组完成的甲状腺手术430例，包括超声刀手术(超声刀组) 230例，传统手术(传统组) 200例。比较两
组的手术时间、术中出血量、腺体切除量、切口长度、术后引流量和术后并发症情况。结果 超声刀组手术时间
为(51.94±3.84) min，传统手术组为(90.88±4.55) min，平均缩短38.94 min，两组比较差异有统计学意义(P＜0.05)，
超声刀组术中出血量为(10.20±1.49) ml，传统手术组为(44.90±4.78) ml，平均减少34.70 ml，两组比较差异有统计
学意义(P＜0.05)，超声刀组手术切口位(4.21±0.86) cm，传统手术组为(9.76±1.10) cm，平均缩短5.55 cm，两组比较
差异有统计学意义(P＜0.05)，其余各项指标比较差异均无统计学意义(P＞0.05)。结论 应用超声刀行开放性甲
状腺手术安全可靠，可明显缩少手术时间，减少术中出血量，缩小手术切口。

【Abstract】 Objective To investigate the value of ultrasonic-harmonic scalpel (UHS) in open hyperthyroid-
ism thyroidectomy. Methods From August 2008 to May 2010, 430 patients were treated with thyroidectomy by the
same operation team, including 230 cases receiving UHS thyroidectomy (UHS group) and 200 cases undergoing con-
ventional thyroidectomy (the conventional group). The operation time, intraoperative bleeding volume, the length of
incision, postoperative draining volume and postoperative complications were compared between the two groups.
Results The mean operating time in the UHS group was (51.94±3.84) min, which is 38.94 min in mean shorter than
that in the conventional group [(90.88±4.55) min]. The mean quantity of intraoperative bleeding volume in the UHS
group was (10.20±1.49) ml, 34.70 ml less than that in the conventional group [(44.90±4.78) ml]. The mean length of
incision in UHS group was (4.21±0.86) cm, 5.55 cm smaller than that in the conventional group [(9.76±1.10) cm].
No statistically significant difference were found in draining and postoperative complications between the two
groups (P>0.05). Conclusion Compared with conventional method, the use of UHS in thyroidectomy is safer and
more effective. It can shorten the length of incision, reduce the operation time and the intraoperative bleeding.
