      标题:基于 IMB模型的睡眠心理行为护理在慢性失眠患者中的应用

      【摘要】 目的 探讨基于信息-动机-行为技巧(IMB)模型的睡眠心理行为护理在慢性失眠患者中的应用效果。方法 选择 2022年 1月至 2023年 5月驻马店市第二人民医院收治的 116例慢性失眠患者展开研究,采用随机数表法将患者均分为对照组和观察组,每组 58例。对照组患者接受常规护理,观察组患者接受基于 IMB模型的睡眠心理行为护理,均护理至出院。比较两组患者护理前后的匹兹堡睡眠质量指数量表(PSQI)、焦虑自评量表(SAS)、抑郁自评量表(SDS)评分和应对方式(面对、回避、屈服)以及简明生活质量指数(SF-36)变化。结果 护理后,两组患者的PSQI评分均降低,且观察组患者的睡眠质量、入睡时间、睡眠时间、睡眠障碍、睡眠效率评分分别为(0.85±0.12)分、(0.82±0.11)分、(0.79±0.10)分、(0.91±0.21)分、(0.82±0.13)分,明显低于对照组的(1.23±0.25)分、(01.31±0.17)分、(1.08±0.21)分、(1.27±0.14)分、(1.28±0.27)分,差异均有统计学意义(P<0.05);护理后,两组患者的SAS、SDS评分降低,且观察组患者的 SAS、SDS评分分别为(29.17±5.11)分、(27.98±3.09)分,明显低于对照组的(35.28±4.09)分、(31.28±3.25)分,差异均具有统计学意义(P<0.05);护理后,两组患者应对方式中的面对评分升高,且观察组为(21.20±3.11)分,明显高于对照组的(18.27±2.84)分,而应对方式中的回避、屈服评分降低,且观察组的回避、屈服评分分别为(7.21±1.23)分、(5.98±0.93)分,明显低于对照组的(9.31±1.08)分、(6.92±1.20)分,差异均具有统计学意义(P<0.05);护理后,两组患者的生活质量评分升高,且观察组患者的躯体功能、物质生活、情感职能、社会功能评分分别为(72.19±4.28)分、(78.23±5.11)分、(79.17±4.32)分、(78.34±4.09)分,明显高于对照组的(67.12±4.10)分、(70.83±4.82)分、(70.42±4.82)分、(70.91±3.90)分,差异均具有统计学意义(P<0.05)。结论 基于IMB模型的睡眠心理行为护理可减轻患者的负面情绪,改善应对方式,提高患者睡眠质量及生活质量,具有临床应用价值。

Application of sleep psycho-behavioral nursing based on IMB model in patients with chronic insomnia.

【Abstract】 Objective To explore the application effect of sleep psycho-behavioral nursing based on the infor-mation-motivatio-behavior skills (IMB) model in patients with chronic insomnia. Methods A total of 116 patients withchronic insomnia admitted to the Second People's Hospital of Zhumadian City from January 2022 to May 2023 wereevenly divided into a control group and an observation group using a random number table method, with 58 cases ineach group. The control group received routine nursing, while the observation group received sleep psycho-behavioralnursing based on the IMB model. The changes in Pittsburgh Sleep Quality Index (PSQI), Self-rating Anxiety Scale(SAS), Self-rating Depression Scale (SDS) scores, coping styles (face, avoid, surrender), and Short Form Quality ofLife Index (SF-36) before and after nursing were compared between the two groups. Results After nursing, the PSQIscores of both groups decreased; the sleep quality, time of falling asleep, sleep time, sleep disorders, and sleep efficien-cy scores of the observation group were (0.85±0.12) points, (0.82±0.11) points, (0.79±0.10) points, (0.91±0.21) points,(0.82±0.13) points, which were significantly lower than (1.23±0.25) points, (01.31±0.17) points, (1.08±0.21) points,(1.27±0.14) points, (1.28±0.27) points in the control group; the differences were statistically significant (P<0.05). Af-ter treatment, the SAS and SDS scores of the two groups decreased; the scores of the observation group were (29.17±5.11) points, (27.98±3.09) points, which were significantly lower than (35.28±4.09) points, (31.28±3.25) points of thecontrol group; the differences were statistically significant (P<0.05). After nursing, the face scores in the two groupsof patients increased, and the score in the observation group was (21.20±3.11) points, which was significantly higherthan (18.27 ± 2.84) points in the control group; the avoidance and surrender scores in the coping styles of the twogroups decreased, and the scores in the observation group were (7.21±1.23) points and (5.98±0.93) points, which weresignificantly lower than (9.31±1.08) points and (6.92±1.20) points in the control group; the differences were statistical-ly significant (P<0.05). After nursing, the quality of life scores of the two groups increased; the physical function, ma-terial life, emotional function, and social function scores of the observation group were (72.19±4.28) points, (78.23±5.11) points, (79.17±4.32) points, and (78.34±4.09) points, which were significantly higher than (67.12±4.10) points,(70.83±4.82) points, (70.42±4.82) points, and (70.91±3.90) points in the control group; the differences were statistical-ly significant (P<0.05). Conclusion Sleep psycho-behavioral nursing based on the IMB model can alleviate nega-tive emotions in patients, improve coping strategies, and enhance their sleep and quality of life, which has clinical ap-plication value.
