      标题:ICU患者耐碳青霉烯类鲍曼不动杆菌感染危险因素的Meta分析

      【摘要】 目的 系统评价医院重症监护室患者感染耐碳青霉烯类鲍曼不动杆菌(CRAB)的危险因素。方法 检索知网、万方、维普、CBM、ScienceDirect、PubMed等中英文数据库,收集国内外 ICU患者发生CRAB感染危险因素病例对照研究、横断面研究及队列研究的相关文献,检索时限从建库至2023年7月1日。完成文献筛选后应用Stata15软件进行Meta分析。结果 排除不符合纳入标准文献,最终纳入33篇文献,其中中文文献29篇,英文文献 4篇,观察组 3 538例,对照组 9 730例。Meta分析结果显示,共有 23个危险因素,其中年龄、性别、侵入性操作5个、基础疾病5个,抗生素治疗药物3个,使用抗生素时间、联合使用抗生素、其他药物使用、ICU住院天数、入院评分、意识状态、机械通气时间超过1周及白蛋白含量低。结论 通过现有结果分析表明,以上诸多危险因素会引起 ICU患者发生CRAB感染。因此,临床科室应结合患者发生感染的相关危险因素进行针对性防治措施,以期减少 ICU患者发生CRAB感染的风险。

【Abstract】 Objective To systematically evaluate the risk factors of carbapenem-resistant Acinetobacter bau-mannii (CRAB) infection in hospital intensive care unit. Methods Chinese and English databases such as CNKI, Wan-fang, VIP, CBM, ScienceDirect, PubMed were searched to collect relevant literature on case-control studies, cross-sec-tion studies, and cohort studies on risk factors for CRAB infection in ICU patients at home and abroad. The search peri-od was from inception to July 1, 2023. After literature screening, meta analysis was performed using Stata15 software.Results Thirty-three literatures were finally included, including 29 Chinese literatures and 4 English literatures, involv-ing 3 538 cases in the observation group and 9 730 cases in the control group. Results of meta-analysis showed a total of23 risk factors, including age, gender, 5 invasive procedures, 5 underlying diseases, 3 antibiotics, duration of antibioticuse, combined use of antibiotics, use of other drugs, length of ICU stay, admission score, state of consciousness, durationof mechanical ventilation longer than 1 week, and low albumin content. Conclusion The above risk factors could causeCRAB infection in ICU patients. Therefore, targeted prevention and control measures should be taken based on the risk factorsrelated to infection in patients in clinical departments, so as to reduce the risk of CRAB infection in ICU patients.
