      标题:膝骨关节炎患者骨髓水肿MRI特征及其与VAS评分的相关性

      【摘要】 目的 分析膝骨关节炎患者骨髓水肿的MRI特征,并探讨其与视觉模拟评分量表(VAS)评分的关系。方法 回顾性分析2021年2月至2022年5月期间于延安市人民医院诊治的85例膝骨关节炎患者的病历资料,根据患者膝关节MRI检查中的骨髓征象分组,其中21例骨髓水肿征象评估为0级者纳入对照组,32例骨髓水肿征象评估为 1级者纳入Ⅰ组,32例骨髓水肿征象评估为 3级者纳入Ⅱ组。比较三组患者的VAS评分,并采用Spearman相关性分析骨髓水肿MRI特征与VAS评分相关性。结果 Ⅰ组、Ⅱ组和对照组患者的VAS评分分别为(5.46±1.03)分、(7.21±1.27)分、(4.13±1.18)分,其中Ⅱ组患者的VAS评分明显高于Ⅰ组和对照组,Ⅰ组明显高于对照组,差异均统计学意义(P<0.05);经 Spearman相关性分析结果显示,骨髓水肿征象等级与VAS评分呈正相关(P<0.05)。结论 股骨关节炎患者经MRI检查图像评估的骨髓水肿征象等级与VAS评分存在相关性,患者骨髓水肿征象等级越高,VAS评分也越高。

MRI characteristics of bone marrow edema in patients with knee osteoarthritis and their relationship with VASscore.

【Abstract】 Objective To analyze MRI characteristics of bone marrow edema in patients with knee osteoarthri-tis, and explore the relationship between MRI characteristics and Visual Analogue Scale (VAS) score. Methods Themedical records of 85 patients with knee osteoarthritis treated in Yan'an People's Hospital between February 2021 andMay 2022 were retrospectively collected. According to bone marrow signs in the MRI examination of the patients' kneejoints, 21 patients with bone marrow edema signs evaluated as Grade 0 were included in the control group, 32 patientswith bone marrow edema signs evaluated as Grade 1 were included in group Ⅰ, and 32 patients with bone marrow ede-ma signs evaluated as Grade 3 were included in Group Ⅱ. VAS scores of the three groups were comparatively analyzed.And the correlation between MRI features of bone marrow edema and VAS score was analyzed through Spearman corre-lation analysis. Results The VAS score was (5.46±1.03) points in groupⅠ, (7.21±1.27) points in group Ⅱ, and (4.13±1.18) points in the control group; the VAS score in GroupⅡwas significantly higher than that in groupⅠand the controlgroup; and the score in group Ⅰ was significantly higher than that in the control group; the differences were statisticallysignificant (P<0.05). Spearman correlation analysis showed that the grade of bone marrow edema sign was positivelycorrelated with VAS score (P<0.05). Conclusion There is a correlation between the grade of bone marrow edema signand VAS score in patients with knee osteoarthritis. The higher the grade of bone marrow edema sign, the higher the VASscore.
