      标题:足跟撕脱皮肤预制皮瓣移植修复足跟撕脱伤

      【摘要】 目的 探讨应用足跟撕脱皮肤于股前外侧预制成皮瓣移植修复足跟负重区撕脱伤创面的手术方法及注意事项。方法 回顾性分析2019年1月至2021年12月兰州手足外科医院急诊收住院治疗的13例足跟皮肤撕脱伤患者的临床资料,将足跟撕脱游离皮肤修薄成全厚皮片于股前外侧区预制成带血运皮瓣,完全成活后游离移植,修复足跟区创面。结果 13例游离移植足跟预制皮瓣全部成活,随访 3~18个月,足跟原有组织结构恢复,皮纹、质底、感觉均基本恢复,与健侧足跟对比无明显差异,外形功能均恢复正常。结论 应用足跟撕脱游离皮肤于股前外侧预制皮瓣,游离移植修复足跟区撕脱伤创面可恢复足跟特殊的组织结构,后期外形功能满意,是解决足跟脱套伤较为可靠的方法之一。

Repair of heel avulsion injury using prefabricated skin flap transplantation with the free heel avulsion skin.

【Abstract】 Objective To investigate the surgical methods and precautions of repairing avulsion wounds inheel bearing area using prefabricated skin flap grafting with the free heel avulsion skin on the anterolateral thigh.Methods The clinical data of 13 patients with heel skin avulsion injury admitted to the Emergency Department of Lan-zhou Hand and Foot Surgery Hospital from January 2019 to December 2021 were retrospectively analyzed. The free skinof the heel avulsion was repaired by prefabrication of the blood flap in the anterolateral thigh area, and free transplanta-tion was performed to repair the heel area wound after complete survival. Results The 13 cases of free heel preformedflaps all survived. After 3-18 months of follow-up, the original tissue structure of the heel was restored, and the skin tex-ture, texture and sensation were basically restored. There was no significant difference between the heel and the healthyside, and the appearance and function of the heel were normal. Conclusion The application of free heel avulsion skinin the prefabricated anterolateral thigh flap and free transplantation to repair the avulsion wound in the heel area can re-store the special tissue structure of the heel, with satisfactory appearance and function in the later stage, which is one ofthe reliable methods to solve the heel degloving injury.
