      标题:鱼骨图分析法结合4M1E法管理在门诊西药房中的应用

      【摘要】 目的 探讨鱼骨图分析法结合人机料法环(4M1E)法管理在门诊西药房中的应用效果。方法 选择2019年2月至2020年2月广州中医药大学第一附属医院药学部门诊西药房中的500张药方为对照组,选择2020年3月至2021年3月上述医院门诊西药房中的500张药方为研究组。对照组实施常规管理,研究组实施鱼骨图分析法结合4M1E法管理。对比两组药方的方剂合格率、管理效果(包括药房人员日调剂量、单方调剂时间、药品盘点时间、患者候药时间)、管理中不良风险事件的发生状况及患者满意度。结果 研究组药方的方剂合格率为98.80%,明显高于对照组的92.60%,差异有统计学意义(P<0.05);研究组药方的药房人员日调剂量为(212.41±68.42)张,明显多于对照组的(109.38±34.08)张,单方调剂时间、患者候药时间、药品盘点时间分别为(2.29±0.57) min、(7.52±2.08) min、(1.24±0.21) d,明显短于对照组的(5.22±1.49) min、(13.63±4.02) min、(3.09±0.82) d,差异均有统计学意义(P<0.05);研究组药方管理中的不良风险事件发生率为6.00%,明显低于对照组的50.00%,差异有统计学意义(P<0.05);研究组药方患者的满意度为 100.00%,明显高于对照组的 82.00%,差异有统计学意义(P<0.05)。结论 鱼骨图分析法结合4M1E法管理在门诊西药房中的应用,能明显提高门诊西药房的方剂合格率及管理水平,减少不良风险事件的发生,提高患者的满意度,值得推广应用。

Application of fishbone diagram analysis combined with man-machine-material-method-environment methodin outpatient western pharmacy.

【Abstract】 Objective To explore the application effect of fishbone diagram analysis combined with man-ma-chine-material-method-environment (4M1E) in Outpatient Western Pharmacy. Methods A total of 500 prescriptions inOutpatient Western Pharmacy, the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicinefrom February 2019 to February 2020 were selected as the control group, and 500 prescriptions in Outpatient WesternPharmacy of the hospital from March 2020 to March 2021 were selected as the study group. The control group ap-plied routine management, while the study group used fishbone diagram analysis combined with 4M1E method. Thequalified rate of prescription, management effect (including daily dispensing amount of pharmacy staff, single dis-pensing time, drug inventory time, and waiting time of patients), occurrence of adverse risk events in management,and patients' satisfaction were compared between the two groups. Results The qualified rate of the prescription in thestudy group was 98.80%, which was significantly higher than 92.60% in the control group (P<0.05). The daily dispens-ing amount of pharmacy staff in the study group was (212.41 ± 68.42) pieces, which was significantly more than(109.38±34.08) pieces in the control group; the single prescription dispensing time, patient waiting time, and drug inven-tory time were (2.29±0.57) min, (7.52±2.08) min, (1.24±0.21) d, which were significantly shorter than (5.22±1.49) min,(13.63±4.02) min, (3.09±0.82) d in the control group (P<0.05); the incidence of adverse risk events in pharmacy manage-ment in the study group was 6.00%, which was significantly lower than 50.00% in the control group (P<0.05); the satis-faction of patients in the study group was 100.00%, which was significantly higher than 82.00% in the control group (P<0.05). Conclusion The application of fishbone diagram analysis method combined with 4M1E method in OutpatientWestern Pharmacy can significantly improve the qualified rate and management level of prescriptions, reduce the occur-rence of adverse risk events, and improve the satisfaction of patients, which is worthy of popularization and application.
