      标题:类风湿关节炎患者PLR、NLR、IL-27水平检测及其与疾病活动度的相关性

      【摘要】 目的 检测类风湿关节炎患者血小板/淋巴细胞比率(PLR)、中性粒细胞/淋巴细胞比值(NLR)、白介素-27 (IL-27)水平,并探讨其与疾病活动度的相关性。方法 回顾性分析2019年7月至2021年7月西安市第五医院风湿免疫科收治的200例类风湿关节炎患者的临床资料(观察组),其中活动期类风湿关节炎170例,缓解期类风湿关节炎 30例;选择同期在西安市第五医院体检的 200例身体健康人群作为对照组。比较两组受检者的PLR、NLR、IL-27水平、28个关节疾病活动度(DAS28)评分,同时比较不同病情类风湿关节炎患者PLR、NLR、IL-27水平和DAS28评分,采用Pearson相关分析法分析PLR、NLR、IL-27水平与DAS28评分的相关性。结果 观察组患者的PLR、NLR、IL-27水平、DAS28评分分别为(3.38±0.45)%、(148.01±30.47)%、(17.49±4.06) pg/mL、(5.42±0.50)分,明显高于对照组的(1.46±0.19)%、(110.25±20.55)%、(5.51±0.40) pg/mL、(1.70±0.08)分,差异均具有统计学意义(P<0.05);活动期类风湿关节炎患者的 PLR、NLR、IL-27水平、DAS28评分分别为(3.13±0.80)%、(160.15±26.55)%、(16.98±2.15) pg/mL、(5.06±0.50)分,明显高于缓解期类风湿关节炎患者的(2.42±0.64)%、(137.22±18.01)%、(10.30±1.03) pg/mL、(2.40±0.18)分,差异均具有统计学意义(P<0.05);经 Pearson相关性分析法分析结果显示,PLR、NLR、IL-27与DAS28评分均呈正相关(r=0.372、0.467、0.805,P<0.05)。结论 类风湿关节炎患者的PLR、NLR、IL-27水平均明显高表达,PLR、NLR、IL-27的表达水平与疾病的活动度密切相关。

Detection of platelet/lymphocyte ratio, neutrophil/lymphocyte ratio, interleukin-27 levels in patients withrheumatoid arthritis and their correlation with disease activity.

【Abstract】 Objective To detect platelet/lymphocyte ratio (PLR), neutrophil/lymphocyte ratio (NLR), and in-terleukin-27 (IL-27) levels in patients with rheumatoid arthritis, and to explore their correlation with disease activity.Methods The clinical data of 200 patients with rheumatoid arthritis admitted to the Department of Rheumatologyand Immunology, Xi'an Fifth Hospital from July 2019 to July 2021 were retrospectively analyzed (observation group),including 170 cases of active rheumatoid arthritis and 30 cases of remission rheumatoid arthritis, and 200 healthy peoplewho received physical examination in the Hospital during the same period were selected as the control group. The levelsof PLR, NLR, IL-27 and disease activity score in 28 joints (DAS28) scores were compared between the two groups, andthe levels of PLR, NLR, IL-27 and DAS28 scores were compared between the patients with different conditions of rheu-matoid arthritis. The correlation between PLR, NLR, IL-27 and DAS28 score was analyzed by Pearson correlation analy-sis. Results The PLR, NLR, IL-27 levels and DAS28 scores of the observation group were (3.38±0.45)%, (148.01±30.47)%, (17.49±4.06) pg/mL, and (5.42±0.50) points, which were significantly higher than (1.46±0.19)%, (110.25±20.55)%, (5.51±0.40) pg/mL, (1.70±0.08) points of the control group (P<0.05). PLR, NLR, IL-27 levels, and DAS28scores of patients with active rheumatoid arthritis were (3.13±0.80)%, (160.15±26.55)%, (16.98±2.15) pg/mL, (5.06±0.50) points, which were significantly higher than (2.42±0.64)%, (137.22±18.01)%, (10.30±1.03) pg/mL, (2.40±0.18) pointsof patients with rheumatoid arthritis in remission (P<0.05). Pearson's correlation analysis showed that PLR, NLR, IL-27were positively correlated with DAS28 score (r=0.372, 0.467, 0.805, P<0.05). Conclusion The levels of PLR, NLRand IL-27 were significantly higher in patients with rheumatoid arthritis, and the expression levels of PLR, NLR, andIL-27 were closely related to disease activity.
