标题：双源CT门静脉成像参数与肝硬化患者肝功能分级的相关性

      【摘要】 目的 探讨双源CT门静脉成像参数与肝硬化患者肝功能分级的相关性。方法 选取2019年5月至2021年5月汉中市中心医院收治的 65例肝硬化患者作为肝硬化组，根据Child-Pugh级别分为A级(n=25)、B级(n=23)和C级(n=17)三组，另取同期来汉中市中心医院行上腹部双源CT检查未发现病灶的健康受试者30例作为对照组，比较肝硬化组与对照组受试者双源CT门静脉成像参数[门静脉主干(MPV)、脾静脉(SPV)、肝内门静脉左支(IHLPV)、肝内门静脉右支(IHRPV)直径]的差异，并比较不同肝功能分级肝硬化患者上述参数的差异，采用Spear-man相关性分析双源CT门静脉成像参数与肝功能分级之间的关系。结果 双源CT门静脉成像结果显示，肝硬化组患者的门静脉主干(MPV)、脾静脉(SPV)、肝内门静脉左支(IHLPV)、肝内门静脉右支(IHRPV)直径明显大于对照组，差异均有统计学意义(P<0.05)；双源CT门静脉成像结果显示，A级、B级和C级患者的MPV、SPV、IHLPV和IHRPV逐渐增大，差异均有统计学意义(P<0.05)，其中C级肝硬化患者MPV、SPV、IHLPV和 IHRPV明显大于A级、B级患者，B级患者大于A级患者，差异均有统计学意义(P<0.05)；经Spearman相关性分析结果显示，肝硬化患者肝功能分级与MPV、SPV、IHLPV、IHRPV均呈正相关性(r=0.692、0.448、0.507和0.563，P<0.05)。结论 双源CT门静成像能清晰显示肝硬化患者门静脉及其侧支循环情况，有助评估患者肝功能损害程度。
