      标题:温针灸对卵巢早衰患者性激素水平和卵巢血流状态的影响

      【摘要】 目的 研究温针灸对卵巢早衰(POF)患者性激素水平和卵巢血流状态的影响。方法 选取2018年9月至2021年3月在安康市中医医院治疗的80例POF患者为研究对象,按照随机数表法将患者分为观察组和对照组,每组40例。对照组患者采用常规西医治疗,观察组患者在对照组治疗的基础上联合温针灸治疗;两组患者疗程均为3个月。比较两组患者的临床治疗效果和不良反应发生情况;比较两组患者治疗前后的卵巢血流状态[卵巢动脉收缩期血流峰值速度(PSV)、阻力指数(RI)、搏动指数(PI)]、性激素水平[促卵泡生成激素(FSH)、促黄体生成激素(LH)、雌二醇(E2)]以及中医证候积分。结果 观察组患者的治疗总有效率为95.00%,明显高于对照组的77.50%,差异有统计学意义(P<0.05);治疗后,两组患者的潮热出汗、失眠、头晕、性欲减退等中医证候积分及总积分均降低,且观察组明显低于对照组,差异均有统计学意义(P<0.05);治疗后,两组患者的血清FSH、LH水平均下降,且观察组分别为(36.45±5.24) IU/L、(22.65±2.85) IU/L,明显低于对照组的(42.72±5.67) IU/L、(30.66±4.21) IU/L,差异均有统计学意义(P<0.05);治疗后,两组患者的E2水平均升高,且观察组为(95.22±11.09) pmol/L,明显高于对照组的(86.22±10.57) pmol/L,差异均有统计学意义(P<0.05);治疗后,两组患者的卵巢中的RI、PI水平均下降,且观察组分别为(0.68±0.09)、(1.40±0.20),明显低于对照组的(0.86±0.12)、(1.76±0.21),差异均有统计学意义(P<0.05);治疗后,两组患者的PSV水平均升高,且观察组为(19.95±2.94) cm/s,明显高于对照组的(17.82±2.17) cm/s,差异均有统计学意义(P<0.05);观察组和对照组患者治疗期间的恶心呕吐、头晕等不良反应总发生率分别为2.50%、5.00%,差异无统计学意义(P>0.05)。结论 温针灸治疗POF可改善患者症状,恢复月经来潮及卵巢功能,临床疗效显著且不增加不良反应。

Influence of needle warm moxibustion on sex hormone level and ovarian blood flow state of patients withpremature ovarian failure.

【Abstract】 Objective To investigate the effects of warm acupuncture on sex hormone levels and ovarian bloodflow in patients with premature ovarian failure (POF).Methods A total of 80 patients with POF treated in Ankang Hos-pital of Traditional Chinese Medicine from September 2018 to March 2021 were selected as the study subjects, whowere divided into a study group and a control group according to random number table method, with 40 patients in eachgroup. Patients in the control group were treated with conventional Western medicine, and patients in the study groupwere treated with warm acupuncture on the basis of the treatment in the control group. The course of treatment in bothgroups was 3 months. The clinical treatment effect and the incidence of adverse reactions were compared between thetwo groups. Ovarian blood flow status [peak systolic blood flow velocity (PSV), resistance index (RI), pulsatile index(PI)], sex hormone levels [follicle-stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2)], and TCM syn-drome score were compared between the two groups before and after treatment. Results The total effective rate ofstudy group was 95.00%, significantly higher than 77.50% of control group (P<0.05). After treatment, hot flashes sweat-ing, insomnia, dizziness, loss of libido, TCM syndrome integral, and total integral were in the two groups in the studygroup were significantly lower than those in the control group (P<0.05). The serum levels of FSH and LH in the twogroups were decreased, and the levels of FSH and LH in the study group were (36.45±5.24) IU/L, (22.65±2.85) IU/L, sig-nificantly lower than (42.72±5.67) IU/L, (30.66±4.21) IU/L in the control group (P<0.05). The level of E2 in the studygroup was (95.22±11.09) pmol/L, significantly higher than (86.22±10.57) pmol/L in the control group (P<0.05). RI, PIlevel in ovaries of the two groups decreased, and the levels in the study group were 0.68±0.09, 1.40±0.20, significantlylower than 0.86±0.12, 1.76 ±0.21 in the control group (P<0.05). After treatment, the level of PSV increased in bothgroups; PSV level in the study group was (19.95±2.94) cm/s, significantly higher than (17.82±2.17) cm/s in the controlgroup (P<0.05). The incidence of adverse reactions such as nausea and vomiting, dizziness showed no significant differ-ence between the two groups (2.50% vs 5.00%, P>0.05). Conclusion Warm acupuncture and moxibustion can improvethe symptoms of POF patients, restore menstruation and ovarian function, which has significant clinical effect and doesnot increase the adverse reactions.
