      标题:胃癌患者血清AFP、MG7-Ag、DKK1、TK1、PGⅠ水平检测及临床意义

      【摘要】 目的 探讨胃癌患者血清甲胎蛋白(AFP)、胃癌相关抗原(MG7-Ag)、分泌型蛋白 Dickkopf-1 (DKK1)、胸苷激酶-1 (TK1)、胃蛋白酶原Ⅰ (PGⅠ)水平检测及临床意义。方法 回顾性分析2018年8月至2019年8月西安市红会医院消化外科收治的 110例胃癌患者(观察组)的临床资料,选择同期我院 85例接受体检的健康人群作为对照组。比较两组受检者的血清AFP、MG7-Ag、DKK1、TK1、PGⅠ水平,并比较不同TNM分期、肿瘤直径、血管浸润程度、淋巴结转移的胃癌患者的血清AFP、MG7-Ag、DKK1、TK1、PGⅠ水平,采用 Spearman相关性分析法分析血清AFP、MG7-Ag、DKK1、TK1、PGⅠ水平与TNM分期、肿瘤直径、血管浸润程度、淋巴结转移的相关性。结果 观察组患者的血清AFP、MG7-Ag、DKK1、TK1水平分别为(24.89±3.71) ng/mL、(7.16±3.05) U/mL、(5.12±1.10) ng/mL、(100.85±20.30) pmol/mL,明显高于对照组的(5.40±0.85) ng/mL、(2.33±0.60) U/mL、(3.05±0.48) ng/mL、(70.00±13.90) pmol/mL,PGⅠ水平为(40.10±7.18) ng/mL,明显低于对照组的(50.57±9.66) ng/mL,差异均有统计学意义(P<0.05);TNM分期高、肿瘤直径大、发生血管浸润和淋巴结转移患者的血清AFP、MG7-Ag、DKK1、TK1、PGⅠ水平明显升高,PGⅠ水平明显降低,差异均有统计学意义(P<0.05);经Spearman相关性分析结果显示,血清AFP、MG7-Ag、DKK1、TK1水平与TNM分期、肿瘤直径、血管浸润程度、淋巴结转移均呈正相关(P<0.05),血清PGⅠ水平与NM分期、肿瘤直径、血管浸润程度、淋巴结转移均呈负相关(P<0.05)。结论 血清AFP、MG7-Ag、DKK1、TK1在胃癌患者中的表达水平明显升高,PGⅠ明显降低,血清AFP、MG7-Ag、DKK1、TK1、PGⅠ的联合检测可为胃癌的临床治疗以及病情的评估提供参考价值。

Detection of serum AFP, MG7-Ag, DKK1, TK1, and PGⅠ levels in patients with gastric cancer and their clinicalsignificance.

【Abstract】 Objective To study the detection of serum levels of alpha-fetoprotein (AFP), monoclonal gastriccancer 7 antigen (MG7-Ag), secreted protein Dickkopf-1 (DKK1), thymidine kinase-1 (TK1), and pepsinogen Ⅰ (PGⅠ) in patients with gastric cancer and their clinical significance. Methods The clinical data of 110 gastric cancer pa-tients (observation group) admitted to the Department of Gastroenterology at Xi'an Honghui Hospital from August 2018to August 2019 were retrospectively analyzed, and 85 healthy people who underwent physical examination in our hospi-tal during the same period were selected as the control group. The serum levels of AFP, MG7-Ag, DKK1, TK1, and PGⅠ of the two groups were compared, as well as the serum levels of AFP, MG7-Ag, DKK1, TK1, and PGⅠ of gastriccancer patients with different TNM stages, tumor diameters, vascular invasion degrees, and lymph node metastasis.Spearman correlation analysis method was used to analyze the correlation between serum levels of AFP, MG7-Ag,DKK1, TK1, and PGⅠ with TNM stage, tumor diameter, vascular invasion, and lymph node metastasis. Results Theserum levels of AFP, MG7-Ag, DKK1, and TK1 of the observation group were significantly higher than those of the con-trol group, i.e., AFP (24.89±3.71) ng/mL vs (5.40±0.85) ng/mL, MG7-Ag (7.16±3.05) U/mL vs (2.33±0.60) U/mL,DKK1 (5.12±1.10) ng/mL vs (3.05±0.48) ng/mL, TK1 (100.85±20.30) pmol/mL vs (70.00±13.90) pmol/mL; and the PGⅠ level was (40.10±7.18) ng/mL in the observation group, which was significantly lower than (50.57±9.66) ng/mL in thecontrol group; all differences were statistically significant (P<0.05). Serum levels of AFP, MG7-Ag, DKK1, TK1, PGⅠin patients with high TNM stage, large tumor diameter, vascular invasion and lymph node metastasis were significantlyincreased, and PGI levels were significantly reduced (P<0.05). Spearman correlation analysis showed that serum levelsof AFP, MG7-Ag, DKK1, and TK1 were positively correlated with TNM stage, tumor diameter, vascular invasion de-gree, and lymph node metastasis (P<0.05); serum PGⅠ level was correlated with NM stage, tumor diameter, vascular in-
