      标题:振幅整合脑电图在新生儿惊厥中的应用价值

      【摘要】 目的 探讨振幅整合脑电图(aEEG)在新生儿惊厥中的应用价值。方法 选择西安市儿童医院2019年1月至2021年8月期间收治的40例新生儿惊厥患儿纳入观察组,同期未合并脑损伤的40例健康新生儿纳入对照组,均行aEEG监测,比较两组新生儿的aEEG监测情况,包括背景活动情况、睡眠觉醒周期(SWC)分布、背景波下边界振幅以及 aEEG分度等。结果 观察组患儿的背景活动轻度异常及重度异常的发生率分别为40.00%和47.50%,明显高于对照组的2.50%和0,差异均有统计学意义(P<0.05);对照组患儿均为成熟的SWC,观察组不成熟SWC及无SWC的发生率分别为70.00%和30.00%,明显高于对照组,差异均有统计学意义(P<0.05);观察组患儿下边界振幅为(3.76±1.02) uV,明显低于对照组的(8.26±2.44) uV,差异有统计学意义(P<0.05);观察组患儿的aEEG轻度异常及重度异常的发生率分别为75.00%和20.00%,明显高于对照组的2.50%和0,差异均有统计学意义(P<0.05)。结论 新生儿惊厥患儿的振幅整合脑电图可出现背景活动、SWC、下边界振幅等的异常改变,有助于新生儿惊厥的早期诊断。

Application value of amplitude integrated electroencephalogram in neonatal convulsion.

【Abstract】 Objective To investigate the application value of amplitude integrated electroencephalogram(aEEG) in neonatal convulsion. Methods Forty children with neonatal convulsion treated in Xi’an Children’s Hospitalfrom January 2019 to August 2021 were included into the observation group. Forty healthy newborns without brain inju-ry in the same period were included into the control group. All children underwent aEEG monitoring. The aEEG monitor-ing of newborns between the two groups were compared, including background activity, sleep wake cycle (SWC) distri-bution, boundary amplitude under background wave, and aEEG division. Results The incidence of mild and severe ab-normalities of background activity in the observation group was 40.00% and 47.50%, respectively, significantly higherthan 2.50% and 0 in the control group (P<0.05). All the children in the control group had mature SWC, and the incidenceof immature SWC and no SWC in the observation group was 70.00% and 30.00%, respectively, significantly higher thanthose in the control group (P<0.05). The lower boundary amplitude in the observation group was (3.76±1.02) uV, whichwas significantly lower than (8.26±2.44) uV in the control group (P<0.05). The incidence of mild and severe aEEG ab-normalities in the observation group was 75.00% and 20.00%, significantly higher than 2.50% and 0 in the control group(P<0.05). Conclusion The amplitude integrated EEG of children with neonatal convulsion can show abnormalchanges in background activity, SWC and lower boundary amplitude, which is helpful for the early diagnosis of neona-tal convulsion.
