      标题:振幅整合脑电图在缺氧缺血性脑病患儿诊断及预后判断中的应用

      【摘要】 目的 探讨振幅整合脑电图(aEEG)在缺氧缺血性脑病(HIE)患儿诊断及预后判断中的应用价值。方法 选择西安市儿童医院2019年1月至2021年4月期间收治的60例HIE患儿纳入观察组,选择同期未合并脑损伤的30例健康新生儿纳入对照组。比较两组新生儿的aEEG异常情况;比较不同病情程度HIE患儿的aEEG异常情况,以及不同程度 aEEG患儿近期预后[2周、4周行新生儿神经行为测定(NBNA)评分]。结果 观察组患儿的 aEEG异常率为63.33%,明显高于对照组的6.67%,差异有统计学意义(P<0.05);aEEG重度异常率在轻度HIE组(4.00%)、中度HIE组(55.00%)及重度HIE组(100.00%)依次上升,差异均有统计学意义(P<0.05);HIE患儿 2周NBNA评分在 aEEG正常组、轻度异常组及重度异常组分别为(38.41±1.22)分、(33.08±5.91)分、(24.19±7.33)分,4周NBNA评分分别为(38.92±1.02)分、(34.17±6.01)分、(25.66±8.21)分,上述评分在三组均依次降低,差异均有统计学意义(P<0.05)。结论 振幅整合脑电图检查可反映缺氧缺血性脑病患儿的病情严重程度,可应用于缺氧缺血性脑病的早期诊断,且对预后的预测具有良好的价值。

Application of amplitude integrated EEG in the diagnosis and prognosis of children with hypoxic-ischemicencephalopathy.

【Abstract】 Objective To investigate the application value of amplitude integrated electroencephalogram(aEEG) in the diagnosis and prognosis of children with hypoxic-ischemic encephalopathy (HIE). Methods The 60 chil-dren with HIE who were treated in Xi'an Children's Hospital from January 2019 to April 2021 were selected to be includ-ed into the observation group, and 30 healthy newborns without brain injury during the same period were selected to beincluded into the control group. The aEEG abnormalities were compared between the two groups of newborns, as well asthe aEEG abnormalities in HIE children with different disease degrees, and the short-term prognosis of children with dif-ferent degrees of aEEG [neonatal behavioral neurological assessment (NBNA) at 2- and 4-week]. Results The abnor-mal rate of aEEG in the observation group was 63.33%, which was significantly higher than 6.67% in the control group(P<0.05). The severe abnormality rate of aEEG increased sequentially in the mild HIE group (4.00%), moderate HIEgroup (55.00% ), and severe HIE group (100.00% ), and the differences were statistically significant (P<0.05). The2-week NBNA score of HIE children in the aEEG normal group, mildly abnormal group, and severely abnormal groupwas (38.41±1.22) points, (33.08±5.91) points, and (24.19±7.33) points, respectively; and the corresponding 4-week NB-NA score were (38.92±1.02) points, (34.17±6.01) points, and (25.66±8.21) points; the above scores decreased in thethree groups, and all differences were statistically significant (P<0.05). Conclusion Amplitude integrated aEEG can re-flect the severity of the condition of children with hypoxic-ischemic encephalopathy, which can be used in the early diag-nosis of hypoxic-ischemic encephalopathy and has good application value in predicting the prognosis.
