      标题:呼出气一氧化氮测定在咳嗽变异性哮喘患儿中的应用

      【摘要】 目的 探讨呼出气一氧化氮(FeNO)测定在咳嗽变异性哮喘(CVA)患儿诊断中的应用价值。方法 选择西安市中心医院儿科于2018年1月至2019年12月期间收治的98例CVA患儿纳入观察组,选择同期在我院体检的 50例健康儿童纳入对照组,比较两组受检儿童以及观察组患儿治疗前后的血清免疫球蛋白E (IgE)及FeNO水平,采用Pearson相关分析法分析FeNO与血清 IgE水平的相关性,同时比较不同哮喘控制水平CVA患儿的血清 IgE及FeNO水平。结果 观察组儿童的血清 IgE及FeNO水平分别为(235.86±50.29) IU/mL、(21.65±6.98) ppb,明显高于对照组的(73.54±22.67) IU/mL、(12.96±3.24) ppb,差异均有统计学意义(P<0.05);经Pearson相关分析结果显示,CVA患儿FeNO与血清 IgE水平呈正相关性(r=0.564,P<0.05);治疗后,观察组患儿的血清 IgE及FeNO水平分别为(112.48±31.65) IU/mL、(15.80±4.32) ppb,均较治疗前明显下降,差异均有统计学意义(P<0.05);治疗后,血清 IgE及FeNO水平在完全控制组、部分控制组、未控制组患儿中依次升高,差异均有统计学意义(P<0.05)。结论 咳嗽变异性哮喘患儿呼出气一氧化氮明显升高,可以有效反映患儿的气道炎症水平及哮喘控制情况,具有较好的应用价值。

Application of fractional exhaled nitric oxide determination in children with cough variant asthma.

【Abstract】 Objective To explore the application value of fractional exhaled nitric oxide (FeNO) determinationin the diagnosis of children with cough variant asthma (CVA). Methods Ninety-eight children with CVA treated in De-partment of Pediatrics, Xi'an Central Hospital from January 2018 to December 2019 were included into the observationgroup, while 50 healthy children were selected as the control group. The levels of serum immunoglobulin E (IGE) andFeNO were compared between the two groups before and after treatment. The correlation between FeNO and serum IgElevels was analyzed by Pearson correlation analysis, while serum IgE and FeNO levels were compared in children withdifferent CVA levels of asthma control levels. Results The levels of serum IgE and FeNO in the observation groupwere (235.86±50.29) IU/mL, (21.65±6.98) ppb, which were significantly higher than (73.54±22.67) IU/mL, (12.96±3.24) ppb in the control group (P<0.05). Pearson correlation analysis showed that FeNO was positively correlated withserum IgE level in children with CVA (r=0.564, P<0.05). After treatment, the levels of serum IgE and FeNO in the ob-servation group were (112.48±31.65) IU/mL, (15.80±4.32) ppb, which were significantly lower than those before treat-ment, with statistically significant difference (P<0.05). After treatment, the levels of serum IgE and FeNO in completecontrol group, partial control group, and uncontrolled group increased in turn, with statistically significant difference(P<0.05). Conclusion Fractional exhaled nitric oxide in children with cough variant asthma is significantly in-creased, which can effectively reflect the situation of airway inflammation level and asthma control in children, andhas good application value.
